乌帕替尼治疗难治性掌跖脓疱病的临床疗效及安全性观察

Efficacy and Safety Observation of Upatinib in Treating Refractory Palmoplantar Pustulosis

目的评估乌帕替尼对常规治疗无效的掌跖脓疱病(palmoplantar pustulosis,PPP)的临床疗效和安全性。方法回顾性分析2022年7月—2023年6月在苏州大学附属第一医院皮肤科门诊因常规治疗无效而接受乌帕替尼治疗的23例PPP患者的临床资料。对PPP患者的皮损面积和严重程度指数(PPPASI)评分、皮肤病生活质量指数(DLQI)评分以及临床医师整体评估(PGA)评分在治疗前后进行了评价,并记录了治疗过程中的不良事件。结果23例PPP患者中,包括6例被诊断为SAPHO综合征的患者,男7例,女16例,平均年龄(34.3±12.8)岁。治疗12周后,PPPASI评分从基线的(12.96±3.32)分降至(6.01±1.98)分,两者比较差异有统计学意义(P<0.05)。DLQI评分从(11.45±5.86)分降至(2.33±1.23)分,差异有统计学意义(P<0.05)。8例患者PGA评分达到了0/1级。在治疗随访期间,未发生严重不良事件。结论乌帕替尼治疗难治性掌跖脓疱病安全有效,可显著提高患者生活质量。

Objective To evaluate the clinical efficacy and safety of uppatinib in patients with palmoplantar pustulosis(PPP)that does not respond to conventional therapy.Methods We retrospectively collected clinical data of 23 patients who received uppatinib due to ineffectiveness of routine treatment in the Dermatology clinic of the First Affiliated Hospital of Soochow University from July 2022 to June 2023.We evaluated the PPP Skin Loss Area and Severity Index(PPPASI)score,the Skin Disease Quality of Life Index(DLQI)score,and the Clinician Global Assessment(PGA)score before and after treatment,and recorded adverse events during treatment.Results In 23 PPP patients,including 6 patients diagnosed with SAPHO syndrome,7 were male and 16 were female with a mean age of(34.3±12.8)years old.After 12 weeks treatment,the PPPASI score decreased from baseline(12.96±3.32)points to(6.01±1.98)points,which was statistically significant(P<0.05).The DLQI score decreased from(11.45±5.86)points to(2.33±1.23)points,and the difference was also statistically significant(P<0.05).Furthermore,eight patients achieved grade 0/1 in the PGA score.No serious adverse events occurred during the treatment and follow-up period.Conclusion The results of this study indicate that oppatinib is safe and effective for the treatment of refractory palmoplantar pustulosis.In addition,upatinib also significantly improved the quality of life of the patients.
