单侧双通道脊柱内镜技术对退变性腰椎间盘突出症的治疗效果评价

Evaluation of the Efficacy of Unilateral Two-channel Spinal Endoscopy in the Treatment of Degenerative Lumbar disc Herniation

目的 探讨针对退变性腰椎间盘突出症(LDH)采用单侧双通道脊柱内镜技术治疗效果。方法 选取2021年2月—2023年1月本院收治的退变性LDH患者80例为研究对象,利用随机数字表法分成观察组以及对照组,每组40例。对照组患者应用经皮内镜椎板间入路椎间盘切除术治疗,观察组患者应用单侧双通道脊柱内镜技术治疗。对比患者的围术期指标、患者治疗前后的腰椎功能与疼痛评分(JOA、ODI、VAS)、术后并发症发生率。结果 观察组患者手术时间与住院时间均比对照组短(P<0.05);术前,两组腰椎功能与疼痛比较,差异无统计学意义(P>0.05);术后3个月,观察组患者JOA评分高于对照组,观察组患者在ODI评分与VAS评分低于对照组(P<0.05);针对并发症统计结果,未见组间发生率的统计学差异(P>0.05)。结论 采取单侧双通道脊柱内镜技术治疗退变性LDH的效果满意,手术时间及住院时间短,可有效改善患者腰椎功能以及缓解疼痛,同时术后并发症发生率低。

Objective To investigate the effect of unilateral double-channel spinal endoscopy for the treatment of degenerative lumbar disc herniation(LDH).Methods A total of 80 patients with degenerative LDH treated from February 2021 to January 2023 were included and divided into observation group and control group by random number table method,with 40 cases in each group.The control group was treated with percutaneous endoscopic interlaminar approach discectomy,while the observation group was treated with unilateral double-channel spinal endoscopy.The perioperative indexes,lumbar function and pain scores(JOA,ODI,VAS) before and after treatment,and the incidence of postoperative complications were compared.Results The operation time and hospital stay in observation group were shorter than those in control group(P< 0.05).At three months after operation,the JOA score of observation group was higher than that of control group,the ODI score and VAS score of observation group were lower than that of control group(P<0.05);As for the statistical results of complications,no statistical difference was found in the incidence of complications between groups(P>0.05).Conclusion Unilateral dual-channel spinal endoscopy is satisfactory in the treatment of degenerative LDH.The operation time and hospital stay are short,which can effectively improve the lumbar function and relieve pain,and the incidence of postoperative complications is low.
