他克莫司软膏联合卤米松乳膏治疗神经性皮炎患者的效果

Effects of Tacrolimus ointment combined with Halometasone cream in treatment of patients with neurodermatitis

目的:观察他克莫司软膏联合卤米松乳膏治疗神经性皮炎患者的效果。方法:选取2021年1月至2023年1月该院收治的150例神经性皮炎患者进行前瞻性研究,按照随机数字表法分为研究组与对照组各75例。对照组采用卤米松乳膏治疗,研究组在对照组基础上联合他克莫司软膏治疗,比较两组治疗总有效率、皮肤屏障功能指标[角质层含水量、皮肤弹性指数(R2)和经表皮失水率(TEWL)]水平、临床症状(局部皮损、皮肤瘙痒和皮肤厚度)积分、血清炎性因子[肿瘤坏死因子-ɑ(TNF-ɑ)、白细胞介素(IL)-1β、IL-6]水平和不良反应发生率。结果:研究组治疗总有效率为92.00%,明显高于对照组的80.00%,差异有统计学意义(P<0.05);治疗后,研究组角质层含水量、R2水平高于对照组,局部皮损、皮肤瘙痒和皮肤厚度等临床症状积分以及TEWL、TNF-α、IL-1β和IL-6水平低于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:他克莫司软膏联合卤米松乳膏治疗神经性皮炎患者可提高治疗总有效率,改善皮肤屏障功能指标水平,降低临床症状积分和炎性因子水平,效果优于单纯卤米松乳膏治疗。

Objective:To observe effects of Tacrolimus ointment combined with Halometasone cream in treatment of patients with neurodermatitis.Methods:A prospective study was conducted on 150 patients with neurodermatitis admitted to the hospital from January 2021to January 2023.According to the random number table method,they were divided into study group and control group,75 cases in each group.The control group was treated with Halometasone cream,while the study group was treated with Tacrolimus ointment on the basis of that of the control group.The total effective rate of treatment,the skin barrier function indexes [stratum corneum water content,skin elasticity index(R2)and transepidermal water loss rate(TEWL)] levels,the clinical symptoms(local skin lesions,skin itching and skin thickness) scores,the serum inflammatory factors [tumor necrosis factor-ɑ(TNF-ɑ),interleukin(IL)-1β,IL-6] levels,and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the study group was 92.00%,which was significantly higher than 80.00% of the control group,and the difference was statistically significant(P<0.05).After the treatment,the stratum corneum water content and the R2 level in the study group were higher than those in the control group;the clinical symptom scores of local skin lesions,skin itching and skin thickness and the levels of TEWL,TNF-α,IL-1β and IL-6in the study group were lower than those in the control group;and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Tacrolimus ointment combined with Halometasone cream in the treatment of the patients with neurodermatitis can improve the total effective rate of treatment,improve the levels of skin barrier function indexes,and reduce the clinical symptom scores and the inflammatory factor levels.Moreover,it is superior to single Halometasone cream treatment.
