消痛愈甲方联合醋酸泼尼松片治疗亚急性甲状腺炎风热痰凝证的有效性及安全性

Efficacy and Safety of Xiaotong Yujia Formula Combined with Prednisone Acetate Tablets in Subacute Thyroiditis with Wind-heat and Phlegm Coagulation Syndrome

目的:探讨消痛愈甲方联合醋酸泼尼松片治疗亚急性甲状腺炎(SAT)风热痰凝证的有效性及安全性。方法:选取2022年3月-2023年8月江阴市中医院收治的SAT患者70例为研究对象,按照随机数字表法分为对照组和观察组,各35例。对照组予以醋酸泼尼松片治疗,观察组在对照组基础上予以消痛愈甲方治疗。比较两组临床疗效、甲状腺功能、甲状腺功能减退发生率、复发情况及不良反应。结果:观察组总有效率高于对照组,差异有统计学意义(P=0.046);治疗后,两组游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)水平降低,促甲状腺激素(TSH)水平升高,观察组FT3、FT4水平低于对照组,TSH水平高于对照组,差异有统计学意义(P<0.05);观察组甲状腺功能减退发生率、复发率均低于对照组,差异有统计意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:消痛愈甲方联合醋酸泼尼松片治疗SAT风热痰凝证的效果确切,可以改善甲状腺功能,减少甲状腺功能减退的发生及复发,安全性好。

Objective:To investigate the efficacy and safety of Xiaotong Yujia formula combined with prednisone acetate tablets in subacute thyroiditis(SAT)with wind-heat and phlegm coagulation syndrome.Methods:Seventy SAT patients in Jiangyin Hospital of Traditional Chinese Medicine were selected from March 2022 to August 2023,and were divided into the control group and ob-servation group according to the randomized numerical table,with thirty-five cases in each group.The control group was treated with prednisone acetate tablets,and the observation group was treated with Xiaotong Yujia formula on the basis of the control group.The clinical efficacy,thyroid function,incidence of hypothyroidism,recurrence and adverse reactions were compared be-tween the two groups.Results:The total effective rate of the observation group was higher than that of the control group,and the dif-ference was statistically significant(P=0.046);after treatment,the levels of free triiodothyronine(FT3)and free thyroxine(FT4)de-creased and the level of thyroid-stimulating hormone(TSH)increased in the two groups,the levels of FT3 and FT4 in the observa-tion group were lower than those in the control group,and the level of TSH was higher than that in the control group,and the differ-ence was statistically significant(P<0.05);the incidence of hypothyroidism and recurrence rate in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05);the difference in the incidence rate of ad-verse reactions between the two groups was not statistically significant(P>0.05).Conclusion:Xiaotong Yujia formula combined with prednisone acetate tablets is effective in treating SAT with wind-heat and phlegm coagulation syndrome,which can improve the thyroid function,reduce the incidence and recurrence of hypothyroidism,and has a good safety profile.
