单期与双期正畸治疗安氏Ⅱ类错[牙合]畸形的临床效果比较

Comparison of clinical effects between single-stage and two-stage orthodontic treatment for An’s class malocclusion

目的比较安氏Ⅱ类错牙合畸形患者采用单期与双期正畸治疗的临床效果。方法选取2021年1月至2023年1月在濮阳油田总医院接受治疗的安氏Ⅱ类错[牙合]畸形患者共计80例,以随机数字表法分为研究组(n=40)与对照组(n=40),对照组实行单期正畸治疗,研究组实行双期正畸治疗,对两组正畸标准指数(PAR)值、面部美学评分、矫治相关参数及咀嚼功能进行比较。结果两组治疗后PAR值下降,且研究组左侧磨牙关系、右侧磨牙关系较对照组更低,差异均有统计学意义(P<0.05);两组治疗后面部美学评分升高,且研究组较对照组更高,差异均有统计学意义(P<0.05);两组治疗后上齿槽座角(SNA)、上下齿槽座角(ANB)、覆盖(Overjet)、覆颌(Overbite)、Y轴角(SGn-SN)、平面角(FMIA)、上下中切牙角(UI-LI)、颌凸角(NA-Pg A)减小,下齿槽座角(SNB)增加,差异均有统计学意义(P<0.05),研究组SNA角、ANB角、Overjet、Overbite、SGn-SN角、NA-Pg A角较对照组更小,SNB角、FMIA角、UI-LI角更大,差异均有统计学意义(P<0.05);两组治疗后咀嚼效率和咬合力提高,且研究组较对照组更高,差异均有统计学意义(P<0.05)。结论与单期正畸治疗相比,双期正畸治疗应用于安氏Ⅱ类错[牙合]畸形中,虽然治疗时间较长,但更能够提高PAR值,改善面部美学及矫治相关参数,提高咀嚼功能。

Objective To compare the clinical effects of single and two-stage orthodontic treatment in patients with An’s class II malocclusion.Methods A total of 80 patients with An’s classⅡmalocclusion who were treated in Puyang Oilfield General Hospital from January 2021 to January 2023 were selected and divided into the study group(n=40)and the control group(n=40)by randomized numerical table method.The control group was treated with single-stage orthodontic treatment,and the study group was treated with two-stage orthodontic treatment.The orthodontic standard index(PAR)values,facial aesthetic scores,orthodontic related parameters,and chewing function in both groups were compared between the two groups.Results After treatment,the PAR values in both groups were decreased,and the differences were statistically significant(P<0.05),and in comparision of the control group,the left and right molar relationships in the study group were lower,and the differences were statistically significant(P<0.05).After treatment,the aesthetic scores in both groups were increased(P<0.05),and in comparision of the control group,the study group was higher,and the differences were statistically significant(P<0.05).The upper alveolar seat angle(SNA),upper and lower alveolar seat angle(ANB),overlay(Overjet),overjaw(Overbite),Y-axis angle(SGn-SN),planar angle(FMIA),upper and lower mesial incisor angle(UI-LI),and mandibular convexity angle(NA-PgA)decreased,and the lower alveolar seat angle(SNB)increased in the two treatment groups,and the differences were statistically significant(P<0.05),and the SNA angle,ANB angle,Overjet,Overbite,SGn-SN´angle,and NA-PgA angle were smaller,and the SNB angle,FMIA angle,and UI-LI angle were larger in the study group compared with the control group,and the differences were statistically significant(P<0.05).After treatment,the chewing efficiency and bite force were improve in both groups after treatment and were higher in the study group than in the control group,and the differences were statistically significant(P<0.05).Conclusion Compared with single-stage orthodontic treatment,two-stage orthodontic treatment applied to An’s Class II malocclusion can increase PAR value,improve facial aesthetics and orthodontic parameters,and enhance chewing function,although the treatment time is longer.
