医护一体化干预对胆囊切除术患者治疗依从性、自我管理效能的影响

Impact of integrated Medical and Nursing Intervention on Treatment Compliance and self-management Efficiency of Patients undergoing Cholecystectomy

目的研究医护一体化干预在胆囊切除患者中的应用效果。方法选取2020年1月至2021年12月期间在南阳医学高等专科学校第一附属医院收治的90例胆囊切除患者为研究对象,按就诊顺序分为对照组(常规护理模式)与研究组(在常规护理模式给予医护一体化干预),各45例。比较两组治疗依从性、胃肠功能恢复情况、自我管理效能[自我效能感量表(GSES)]及负性情绪[焦虑自评量表(SAS)、抑郁自评量表(SDS)]。结果研究组治疗依从性(100.00%)高于对照组(86.67%),差异有统计学意义(P<0.05)。研究组肛门恢复排气时间、肠鸣音恢复时间、肛门恢复排便时间均短于对照组,差异有统计学意义(P<0.05)。出院时,两组GSES评分均明显升高,且研究组高于对照组,差异有统计学意义(P<0.05)。出院时,两组SAS、SDS评分均明显降低,且研究组低于对照组,差异有统计学意义(P<0.05)。结论医护一体化干预可确保胆囊切除患者治疗依从性,改善负性情绪,提高自我管理效能,促进胃肠功能恢复。

Objective To study the application effect of integrated medical and nursing intervention in patients with cholecystectomy.Methods A total of 90 patients with cholecystectomy admitted to the First Affiliated Hospital of Nanyang Medical College from January 2020 to December 2021 were selected as the study subjects.They were divided into control group(conventional nursing mode)and study group(integrated medical and nursing intervention under conventional nursing mode)according to the order of seeing a doctor,with 45 cases in each group.Treatment compliance,gastrointestinal function recovery,self-management efficacy[General Self-Efficacy Scale(GSES)],and negative emotions[Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS)]between two groups were compared.Results The treatment compliance of the study group(100.00%)was higher than that of the control group(86.67%),and the difference was statistically significant(P<0.05).The study group had shorter anal recovery time,bowel sounds recovery time,and anal defecation recovery time compared to those in the control group,with statistically significant differences(P<0.05).At discharge,the GSES scores of both groups significantly increased,and the study group was higher than the control group,with a statistically significant difference(P<0.05).At discharge,the SAS and SDS scores of both groups significantly decreased,and the study group was lower than the control group,with a statistically significant difference(P<0.05).Conclusion Integrated medical and nursing intervention can ensure treatment compliance,improve negative emotions,enhance self-management efficiency,and promote gastrointestinal function recovery in patients undergoing cholecystectomy.
