蜂针疗法治疗膝骨关节炎临床观察

Clinical observation on treatment of knee osteoarthritis with bee venom therapy

目的观察蜂针疗法对膝骨关节炎(KOA)患者的临床疗效,为中医治疗KOA提供新方法。方法选取2020年1月至2023年6月绍兴市中心医院及宁波市第二医院中医科门诊患者150例肝肾亏虚型KOA患者为研究对象,按照随机数表法将患者分为观察组与对照组,每组各75例。观察组运用蜂针疗法调理;对照组运用塞来昔布胶囊200 mg,每日1次,餐后口服,氨基葡萄糖片0.75 g,每日2次,餐中口服。两组疗程均为5周。比较两组治疗前后西安大略麦克马斯特大学骨关节炎指数(WOMAC)评分、关节压痛视觉模拟量表(VAS)评分、50 m行走后膝关节压痛VAS评分与关节肿胀VAS评分。结果共有137例患者完成本研究,其中观察组70例,对照组67例。观察组治疗后的总有效率(82.86%)显著高于对照组(71.64%),差异有统计学意义(P<0.05)。治疗后两组关节肿胀VAS评分、50 m行走后膝关节压痛VAS评分、关节压痛VAS评分及WOMAC评分与治疗前相比明显降低,差异有统计学意义(P<0.05)。治疗后观察组关节肿胀VAS评分、50 m行走后膝关节压痛VAS评分、关节压痛VAS评分及WOMAC评分与对照组比较均显著降低,差异有统计学意义(P<0.05)。结论蜂针疗法在临床中可以明显改善KOA相关症状,并且比氨基葡萄糖片与塞来昔布胶囊具有更显著的临床效果。

Objective To observe the clinical efficacy of bee venom therapy on patients with knee osteoarthritis(KOA),so as to provide a new method for treating knee osteoarthritis with traditional Chinese medicine.Methods A total of 150 patients with KOA with liver and kidney deficiency who were admitted to the clinic of the Department of Traditional Chinese Medicine in Shaoxing Central Hospital and Ningbo No.2 Hospital from January 2020 to June 2023 were selected as the research object and randomly divided into the observation group and the control group,with 75 patients in each group.The treatment group was treated with bee venom therapy,while the control group was given celecoxib capsule(200 mg,qd,po,after meals);glucosamine tablets(0.75 g,bid,po,during meals).The course of treatment in both groups was 5 weeks.Before and after treatment,the scores of Western Ontario and McMaster University osteoarthritis index(WOMAC),score of visual analogue scale(VAS)of joint tenderness,VAS score of knee tenderness after 50-m walking and VAS score of joint swelling were compared between the two groups.Results A total of 137 patients completed this study,including 70 patients in the observation group and 67 patients in the control group.The total effective rate of the treatment group(82.86%)was significantly higher than that of the control group(71.64%),with statistically significant difference(P<0.05).After treatment,the VAS score of joint swelling,the VAS score of knee tenderness after 50-m walking,the VAS score of joint tenderness and the WOMAC score of the two groups were significantly lower than those before treatment,with statistically significant differences(P<0.05).Aft er treat ment,t he VAS score o f joi nt swelli ng,t he VAS sc ore of knee tenderness after 50-m walking,the VAS score of joint tenderness and the WOMAC score in the observation group were significantly lower than those in the control group,with statistically significant differences(P<0.05).Conclusion Bee venom therapy can obviously improve KOA-related symptoms in clinic,and it has more significant clinical effect than glucosamine tablets and celeco xib capsules.
