摘要
肝细胞癌行肝移植后出现胆总管复发罕见, 且针对此情况的治疗方式更是鲜有参考。本文报告了1例肝细胞癌行肝移植治疗后出现胆总管复发并接受根治性胰十二指肠切除术的病例, 总结了其首次来我院就诊时未能诊断恶性胆道狭窄的原因, 分析了复发的可能原因, 并探讨了手术过程中如何避免胆总管复发以及复发后的治疗方法。希望这些经验可以为临床实践提供有益的参考。
There are very few reported cases of biliary duct recurrence after liver transplantation for hepatocellular carcinoma,and treatment options for this condition are scarce.Here,we report a case of biliary duct recurrence after liver transplantation for hepatocellular carcinoma,which was subsequently treated with radical pancreatoduodenectomy.The diagnosis and treatment process of this patient were reviewed,the reasons why the malignant biliary duct stricture was not diagnosed when he first came to our hospital were summarized,the possible causes of recurrence were analyzed,and the methods to prevent biliary duct recurrence during surgery and treatment options after recurrence were discussed.We hope that these experiences can provide valuable references for clinical practice.
