卵巢癌肝转移的外科治疗进展

目前转移性肝癌是肝脏外科关注的一个焦点, 虽然结直肠癌肝转移和神经内分泌肿瘤肝转移患者已经获得了良好结局, 但卵巢癌肝转移(OCLM)因特殊的肿瘤特征和转移方式的多样化给治疗带来了极大的挑战。肝脏外科医生与妇科医生的观点和技术不同导致对疾病的评估和治疗具有差异性, 以往认为卵巢癌发生肝转移不适合手术。近年来各种诊疗技术不断提高, 尤其是多学科诊疗制度的建立极大地保证了OCLM患者的手术安全, 明显改善了预后。当前对于OCLM患者, 临床依旧秉持积极手术治疗的态度。本文就OCLM的转移方式、特征和外科治疗等最新进展进行综述。

Metastatic liver cancer is now a focus of attention in liver surgery.The patients with colorectal cancer liver metastasis and neuroendocrine tumor liver metastasis have already achieved good outcomes,while ovarian cancer liver metastasis(OCLM)still faces great challenges to treatment due to their special tumor characteristics and diverse metastasis methods.The different perspectives and techniques of liver surgeons and gynecologists lead to differences in the evaluation and treatment of diseases.Previously,it was believed that OCLM was no longer suitable for surgery.However,in recent years,various diagnosis and treatment technologies have been continuously improved,especially the establishment of multidisciplinary diagnosis and treatment system.Through the cooperation of multiple disciplines,patients can obtain individualized treatment schemes,which greatly ensures the surgical safety of OCLM patients and significantly improves their prognosis.Currently,we still maintain an active surgical treatment attitude towards OCLM patients.This article reviewed the metastasis,characteristics and surgical treatment of OCLM.
