中性粒细胞/淋巴细胞比值联合白细胞介素6对新型冠状病毒肺炎重症患者的预测

Neutral-to-lymphocyte Ratio Combined with Interleukin-6 for Predicting the Prognosis of Severe COVID-19 Patients

目的评估中性粒细胞/淋巴细胞比值(NLR)及白细胞介素6(IL-6)对新型冠状病毒肺炎重症患者的预测价值。方法选取苏州市相城人民医院于2023年1月收治的符合入组标准的40例新型冠状病毒肺炎重症患者,并按照1:1比例随机挑选40例非重症患者。收集患者的临床资料、外周血常规、细胞因子检测及血凝检查等结果,并比较分析。结果两组患者在NLR、PLR、CRP、IL-6、PT、INR的比较中,差异具有统计学意义(P均<0.05)。进一步二元Logistic回归分析后发现,NLR(OR=1.220,95%CI:1.040~1.431,P=0.014)、IL-6(OR=1.101,95%CI:1.034~1.171,P=0.002)是新型冠状病毒肺炎患者发展为重症的独立危险因素。ROC曲线分析结果显示,NLR、IL-6及两指标联合的AUC、敏感度、特异度分别为(0.711,0.832,0.899)、(70.0%,75.0%,90.0%)、(70.0%,97.5%,97.5%)。结论NLR及IL-6对预测新型冠状病毒肺炎患者发展为重症具有良好的预测价值,可对临床工作提供帮助。

Objective To evaluate the predictive value of neutral-to-lymphocyte ratio(NLR)and interleukin-6(IL-6)for severe COVID-19 patients.Methods 40 severe COVID-19 patients who met the inclusion criteria and 40 non-severe patients were randomly selected from Suzhou Xiangcheng People's Hospital in January 2023 at a ratio of 1:1.Clinical data,peripheral blood routine,cytokine detection,and coagulation examination results were collected and analyzed.Results There were statistically significant differences between the two groups in NLR,PLR,CRP,IL-6,PT,and INR(all P<0.05).Further binary logistic regression analysis showed that NLR(OR=1.220,95%CI:1.040-1.431,P=0.014)and IL-6(OR=1.101,95%CI:1.034-1.171,P=0.002)were independent risk factors for severe COVID-19.The ROC curve analysis showed that the AUC,sensitivity,and specificity of NLR,IL-6,and combined detection of both were 0.711,0.832,0.899;70.0%,75.0%,90.0%;and 70.0%,97.5%,97.5%,respectively.Conclusion NLR and IL-6 have good predictive value for predicting severe COVID-19 patients,which can provide assistance to clinical work.
