TRIM59在类风湿关节炎患者血清中的表达水平及临床意义

Expression Level and Clinical Significance of TRIM59 in Serum of Patients with Rheumatoid Arthritis

目的通过对类风湿关节炎(RA)患者血清TRIM59水平检测,分析TRIM59与RA疾病活动度的关系,探讨其表达水平在RA中的临床意义。方法选取2021年11月1日—2023年01月31日在苏州大学附属第一医院风湿科诊治的118例RA病人,作为RA组。根据病情活动度(DAS28评分-CRP),将RA组分为病情缓解组、低度活动组、中度活动组、高度活动组。另外选取同期健康体检者50名作为健康对照组。采用酶联免疫吸附法(ELISA)分别检测以上所有受试者血清中TRIM59的表达水平,比较各组间TRIM59水平差异,并分析其与RA各项指标之间的相关性。结果1.RA组患者血清中的TRIM59表达水平明显低于健康对照组,差异具有统计学意义(P<0.001);2.病情缓解组[(36.75±7.86)ng/L]、低度活动组[(28.46±10.33)ng/L]、中度活动组[(18.76±5.04)ng/L]、高度活动组[(12.18±1.86)ng/L]的血清TRIM59表达水平差异具有统计学意义(P<0.001);3.血清TRIM59水平与ESR、CRP、晨僵时间、抗CCP抗体、DAS28评分存在负相关性(P均<0.05),与年龄、RF无相关性(P均>0.05)。结论1.相较于健康体检者,RA患者血清中TRIM59水平降低,并且该水平随着RA患者病情活动而降低,提示其可能与RA疾病活动度之间存在密切联系。2.TRIM59的血清表达水平与ESR、CRP、晨僵时间、抗CCP抗体水平、DAS28评分成负相关,提示TRIM59有望成为RA患者疾病活动或进展的监测指标,为RA的早期干预和靶向治疗提供新的思路。

Objective To discuss and analyze the relationship between TRIM59 and disease activity of rheumatoid arthritis(RA)by detecting the level of TRIM59 in serum of patients with RA,and the clinical significance of its expression level.Methods 118 patients with RA treated in the Rheumatology Department of First Affiliated Hospital of Soochow University from November 1,2021 to January 31,2023 were selected as the RA group.According to disease activity(DAS28 score-CRP),RA group was divided into remission group,low activity group,moderate activity group and high activity group.In addition,50 healthy subjects were selected as healthy control group.The expression level of TRIM59 in serum of all the above subjects was detected by enzyme-linked immunosorbent assay(ELISA),and the difference of TRIM59 level among all groups was compared,and the correlation between TRIM59 level and RA indexes was analyzed.Results(1)The expression level of TRIM59 in serum of RA patients was significantly lower than that of healthy control group,and the difference was statistically significant(P<0.001).(2)The expression level of TRIM59 in serum of remission group[(36.75±7.86)ng/L],low activity group[(28.46±10.33)ng/L],moderate activity group[(18.76±5.04)ng/L],and high activity group[(12.18±1.86)ng/L]was statistically significant(P<0.001).(3)Serum TRIM59 level was negatively correlated with ESR,CRP,anti-CCP antibody,morning stiffness and DAS28 score(P<0.05).There was no correlation between serum TRIM59 level with age and RF(P>0.05).Conclusion(1)Compared with healthy subjects,the level of TRIM59 in serum of RA patients decreased,and this level decreased with the disease activity of RA patients,suggesting that it may be closely related to the disease activity of RA.(2)The serum expression level of TRIM59 was negatively correlated with ESR,CRP,anti-CCP antibody,morning stiffness and DAS28 score,suggesting that TRIM59 is expected to be a monitoring index for disease activity or progression in RA patients,providing a new basis for early intervention and targeted therapy in RA patients.
