连续性血液滤过对重症急性肾衰竭患者血流动力学及炎症因子的影响

Continuous Hemofiltration's Impact on Hemodynamics and Inflammatory Markers in Patients with Severe Acute Renal Failure

摘要 目的探究连续性血液滤过在重症急性肾衰竭患者中对血流动力学和炎症因子的作用。方法研究纳入2021年6月—2023年6月万安县人民医院收治的76例重症急性肾衰竭患者,通过随机数表法被分为对照组与观察组,各38例,对照组采用间歇性血液滤过,观察组实施连续性血液滤过治疗,比较两组肾功能[血尿素氮(BUN)、血肌酐(Scr)]、血流动力学[中心静脉压(CVP)、平均动脉压(MAP)、心率(HR)、心脏指数(CI)]、炎症因子[白细胞介素-6(IL-6)]及并发症发生率。结果治疗前,两组患者的BUN、Scr、CVP、MAP、HR、CI、IL-6相比,差异无统计学意义(P>0.05);治疗后,观察组的BUN、Scr、CVP、HR、IL-6水平及并发症发生率均显著低于对照组;观察组MAP、CI均较对照组高,差异有统计学意义(P<0.05)。结论连续性血液滤过在治疗重症急性肾衰竭患者方面表现出显著成效,它不仅能有效优化患者的血流动力学状况,还能显著减少炎症因子的水平,并有助于降低并发症的风险。 Objective To investigate the effects of continuous blood filtration on hemodynamics and inflammatory factors in patients with severe acute renal failure.Methods This study included 76 patients with severe acute renal failure treated at Wan'an County People's Hospital from June 2021 to June 2023.They were randomly divided into control group and observation group,with 38 patients in each.The control group received intermittent blood filtration,while the observation group underwent continuous blood filtration treatment.The study compared renal function[blood urea nitrogen(BUN),serum creatinine(Scr)],hemodynamics[central venous pressure(CVP),mean arterial pressure(MAP),heart rate(HR),cardiac index(CI)],inflammatory factors[interleukin-6(IL-6)],and the incidence of complications between the two groups.Results Before treatment,there were no significant differences in BUN,Scr,CVP,MAP,HR,CI,and IL-6 levels between the two groups(P>0.05).After treatment,the observation group showed significantly lower levels of BUN,Scr,CVP,HR,IL-6,and a lower incidence of complications compared to the control group.The observation group also had higher MAP and CI,with statistically significant differences(P<0.05).Conclusion Continuous blood filtration demonstrates significant effectiveness in treating patients with severe acute renal failure.It not only effectively optimizes hemodynamic conditions but also significantly reduces the levels of inflammatory factors,thereby helping to lower the risk of complications.
