高频重复经颅磁刺激联合氟哌噻吨美利曲辛片治疗脑卒中后抑郁的临床研究

Effects of High-frequency Repetitive Transcranial Magnetic Stimulation Combined with Flupentixol and Melitracen Tablets on Patients with Post-Stroke Depression

目的研究高频重复经颅磁刺激(rTMS)联合氟哌噻吨美利曲辛片对脑卒中后抑郁的治疗效果。方法选取威海市中心医院神经内科2022年1-10月收治的47例脑卒中后抑郁患者,将患者随机分为研究组和对照组,对照组23例,研究组24例。对照组给予氟哌噻吨美利曲辛片每日早午各1片口服,治疗4周。研究组在此基础上,增加高频重复经颅磁刺激治疗,治疗前和治疗4周后采用汉密尔顿抑郁量表(HAMD)评定抑郁状况、美国国立卫生研究院卒中量表(NIHSS)评定神经功能康复状况、匹兹堡睡眠质量指数量表(PSQI)评定睡眠情况。结果治疗4周后,研究组患者的HAMD、NIHSS和PSQI评分明显低于对照组,差异有统计学意义(P<0.05)。结论高频重复经颅磁刺激联合氟哌噻吨美利曲辛片可有效缓解卒中后抑郁症状,促进神经功能的康复,提高睡眠质量及生活质量。

Objective To investigate the effect of high-frequency repetitive transcranial magnetic stimulation(rTMS)combined with flupentixol melitracen tablets on patients with post-stroke depression.Methods A total of 47 patients with poststroke depression admitted to the Department of Neurology of Weihai Central Hospital from January 2022 to October 2022 were selected and randomly divided into study group and control group,with 23 cases in the control group and 24 cases in the study group.The control group was given flupentixol melitracen tablet orally,one tablet at morning and one tablet at noon every day,for 4 weeks.On this basis,the study group added high-frequency repetitive transcranial magnetic stimulation plus treatment.Depression status was assessed by the Hamilton Depression Scale(HAMD)before treatment and 4 weeks after treatment,neurological recovery status was assessed by the National Institutes of Health Stroke Scale(NIHSS),and sleep status was assessed by the Pittsburgh Sleep Quality Index Scale(PSQI).Results After 4 weeks of treatment,HAMD,NIHSS and PSQI scores of the study group were significantly lower than those of the control group,the difference was statistically significant(P<0.05).Conclusion High-frequency repetitive transcranial magnetic stimulation combined with flupentixol melitracen tablets can effectively relieve depression symptoms,promote neurological rehabilitation,and improve sleep quality and quality of life in patients with post-stroke depression.
