从“阳化气,阴成形”论苍附导痰汤加减治疗单纯性肥胖症

Treatment of Simple Obesity with Modified Cangfu Daotan Decoction Based on Theory of“Yang Transforms into Qi,and Yin Shapes up Body”

肥胖发病率高,已成为中国重大的公共卫生问题。“阳化气,阴成形”理论出自《素问·阴阳应象大论》,其与肥胖发生的机制密切相关。当“阳化气”不足、“阴成形”太过时肥胖则可发生,此基于形气的视角与中医脏腑辨证的角度均提示痰湿是肥胖的重要病机之一。苍附导痰汤加减有祛痰除湿、调畅气机之功,可缓解“阴成形”太过,助“阳化气”,与肥胖患者多痰湿之病机相合,临床上以此方治疗单纯性肥胖症效果优良。本文从“阳化气,阴成形”的理论出发,结合吾师叶丽芳主任临床病案论述以苍附导痰汤加减治疗单纯性肥胖症,以期为治疗肥胖症提供新的思路及方向。

The high incidence of obesity has become a major public health problem in China.The theory of“Yang transforms into Qi,and Yin shapes up the body”comes from The Great Treatise on the Interaction of Yin and Yang in the Plain Question,which is closely related to the mechanism of obesity.Obesity can occur when Yang insufficiently transforms into Qi,and Yin excessively shapes up the body.This perspective based on the body and Qi and the TCM syndrome differentiation of zangfu viscera indicate that phlegm-dampness is one of the important pathogenesis of obesity.Modified Cangfu Daotan Decoction has the function of expelling phlegm,removing dampness,and regulating the Qi,which can alleviate the phenomenon of“Yin excessively shapes up the body”and promote Yang to transform into Qi.For the pathogenesis of excess phlegm and dampness in obese patients,Modified Cangfu Daotan Decoction has a good clinical effect on the treatment of simple obesity.According to the theory of“Yang transforms into Qi,and Yin shapes up the body”and the clinical case of my mentor,Ye Lifang,the treatment of simple obesity with Modified Cangfu Daotan Decoction was discussed in this paper,so as to provide new ideas and directions for the treatment of obesity.
