《针灸大成》肩井穴的应用探析

Clinical Application of Jianjing(GB21)in Great Compendium of Acupuncture and Moxibustion(Zhen Jiu Da Cheng)

本文整理《针灸大成》中肩井穴的文献记载及现代应用,对其在定位、操作方法和临床应用等方面归纳总结后进行分析。该穴可针可灸,但针刺深度不宜过深,且孕妇禁针;对其针或灸具有疏通经络、调和气血和通调肝胆气机等作用;肩井穴的临床治疗范围较为广泛,可用于治疗肩臂等局部的疾患,头胸部、腰背部等经脉所过之处的疾患及瘰疬等特殊疾患,临证需依据患者形体和疾病的轻重缓急加以灵活变通,但还有部分主治作用未能得到较好的运用,值得进一步研究探索与验证,并在临床中推广。

By reviewing the records of Jianjing(GB21)in the Great Compendium of Acupuncture and Moxibustion(Zhen Jiu Da Cheng)and modern application of this acupoint,we summarized the positioning,operation methods,and clinical application of this acupoint.Acupuncture and moxibustion can be applied at Jianjing(GB21),but the acupuncture should not be too deep and should be prohibited in pregnant women.Acupuncture or moxibustion at this acupoint has the effects of dredging meridians and collaterals and regulating qi movement(especially in the liver and gallbladder)and blood.Jianjing(GB21)can be employed to treat a wide range of diseases,including local diseases in the shoulder and arm,diseases in the head,chest,and lower back where meridians go through,and special diseases such as scrofula.The treatment method should be adjusted flexibly in clinical practice considering the patient's physical condition and the disease severity.Some of the main therapeutic effects of this acupoint have not been fully exerted,which is worth further research,verification,and promotion in clinical practice.
