北京市某三级医院高血压住院患者费用及其影响因素分析

Analysis of Hospitalization Expenses and Influencing Factors of Inpatients with Hypertension in a Tertiary Hospital in Beijing

目的探讨北京市某三级医院高血压住院患者费用构成及其影响因素,为高血压患者出院费用能有效的控制提供参考借鉴。方法选取北京市某三级医院病案系统2021年1月1日-12月31日出院12401例住院高血压患者的总费用及构成情况,采用单因素方差分析和多元逐步回归分析法对住院费用的影响因素进行统计分析。结果12401例高血压出院患者人均住院费用为23334.37元,西药费占比居于首位,达到38.25%;手术费和治疗费次之,分别占比36.41%和25.34%。多因素分析结果显示高血压分级、年龄、性别、住院天数是影响高血压住院患者住院费用的主要影响因素,其中高血压分级对出院费用影响最大,比较高血压3级和高血压1级的出院患者,3级患者平均住院费用增加8322.23元,均P<0.05。结论高血压具有较高的致残率,婚姻、年龄等人口学信息为不可控因素,而是否有并发症发生、是否手术等为可控因素,医院要全面提高医疗技术水平,做到早发现、早治疗,科学控制医疗费用的支出,从而有效地减轻社会经济负担和医疗消耗资源。

Objectives This study aims to explore the cost composition and influencing factors of hypertensive inpatients in a tertiary hospital in Beijing,in order to provide a reference for effective control of discharge costs for hypertensive patients.Methods The total costs and composition of 12,401 hospitalized hypertensive patients discharged from the medical record system of a tertiary hospital in Beijing from January 1 to December 31,2021 were selected.Single-factor analysis of variance and multiple stepwise regression analysis were used to conduct statistical analysis on the influencing factors of hospitalization costs.Results The per capita hospitalization expense of 12,401 discharged patients with hypertension was 23,334.37 yuan,with western medicine expenses accounting for the largest proportion,reaching 38.25%.Surgery fees and treatment fees followed,accounting for 36.41%and 25.34%respectively.Multi-factor analysis results showed that hypertension grade,age,gender,and length of stay were the main factors affecting the hospitalization costs of inpatients with hypertension.Among them,the grade of hypertension had the greatest impact on discharge costs.Comparing discharged patients with grade 3 hypertension and grade 1 hypertension,the average hospitalization cost of grade 3 patients increased by 8322.23 yuan(both P<0.05).Conclusions Hypertension has a high disability rate and requires long-term drug treatment to control,placing a greater financial burden on patients and families.It is necessary to achieve early detection and early treatment and scientifically control medical expenditures,thereby effectively reducing social and economic burdens and medical resource consumption.
