失效模式与效应分析在食管胃底静脉曲张内镜下食管胃底静脉曲张精准断流术后患者中的应用效果

Application effect of failure mode and effect analysis in the patients after endoscopic selective varices devascularization for esophageal and gastric varices

目的探究失效模式与效应分析在食管胃底静脉曲张内镜下食管胃底静脉曲张精准断流术(ESVD)后患者中的应用效果。方法选取2022年1月至11月九江学院附属医院收治的101例食管胃底静脉曲张ESVD后患者作为研究对象,采用随机数字表法分为对照组(50例)和观察组(51例)。对照组进行常规护理干预,观察组进行失效模式与效应分析为指导的护理干预。比较两组的住院时间、并发症发生率、满意度、护理干预前后的疼痛情况[数字评价量表(NRS)]及情绪状态[焦虑自评量表(SAS)及抑郁自评量表(SDS)]。结果观察组的住院时间短于对照组,并发症总发生率低于对照组,满意度高于对照组,差异有统计学意义(P<0.05);两组护理干预前的NRS评分、SAS评分及SDS评分比较,差异无统计学意义(P>0.05),观察组护理干预1、3、5 d的NRS评分、SAS评分及SDS评分低于对照组,差异有统计学意义(P<0.05)。结论失效模式与效应分析在食管胃底静脉曲张ESVD后患者中的应用效果较好,可缩短患者的住院时间及降低并发症发生率,同时对患者的疼痛及不良情绪也有较好的改善作用,更受患者认可。

Objective To investigate the application effect of failure mode and effect analysis in the patients after endoscopic selective varices devascularization(ESVD)for esophageal and gastric varices.Methods A total of 101 patients with esophageal and gastric varices after ESVD admitted to Jiujiang University Affiliated Hospital from January to November 2022 were selected as the study objects,and they were divided into control group(50 cases)and observation group(51 cases)by random number table method.The control group received routine nursing intervention,and the observation group received nursing intervention guided by failure mode and effect analysis.The duration of hospital stay,complication rate,satisfaction rate,pain(numerical rating scale[NRS])and emotional state(self-rating anxiety scale[SAS]and self-rating depression scale[SDS])before and after nursing intervention were compared between the two groups.Results The duration of hospital stay in the observation group was shorter than that in the control group,the total complication rate was lower than that in the control group,and the satisfaction rate was higher than that in the control group,the differences were statistically significant(P<0.05).There were no significant differences in NRS,SAS and SDS score between the two groups before nursing intervention(P>0.05).The NRS,SAS and SDS score of the observation group at 1,3,5 d of nursing intervention were lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusion The application of failure mode and effect analysis in patients with esophageal and gastric varices after ESVD has a good effect,which can shorten the duration of hospital stay of patients and reduce the incidence of complications,and also has a good effect on the pain and adverse emotions of patients,which is more recognized by patients.
