彩色多普勒超声对甲亢患者甲状腺血流分级的评估价值及与甲状腺激素的相关性

Evaluation value of color Doppler ultrasonography in thyroid blood flow grading of hyperthyroid patients and its correlation with thyroid hormone

目的 探讨彩色多普勒超声对甲亢患者甲状腺血流分级的评估价值及与甲状腺激素的相关性。方法 本研究共收治了110例甲亢患者,选取时间为2021年1月至2022年4月,病例来源为安阳市第三人民医院,进行回顾性分析,做131I治疗前采用彩色多普勒超声对患者进行诊断,并对患者的甲状腺激素指标进行检测。采用纵横断面的彩色多普勒超声显像评定甲状腺的血流分级情况,将其分为1级和2级,分析甲状腺影像学检查血流分级与各项血清甲状腺激素相关指标之间的关系。结果 甲状腺血流分级为1级、2级患者的游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺素受体抗体(TRAb)指标之间比较,差异均有统计学意义(均P<0.05),且上述指标与甲状腺血流分级之间均呈正相关(r值=0.42、0.41、0.24,P=0.001、0.003、0.029),甲状腺血流分级越高,则FT3、FT4、TRAb指标水平越高(P=0.001、0.002、0.027)。结论 甲亢患者甲状腺血流分级为1级、2级患者的FT3、FT4、TRAb指标之间差异均有统计学意义,且上述指标与甲状腺血流分级之间均呈正相关,即彩色多普勒超声血流分级和甲状腺激素之间存在联系,在临床诊断和治疗甲亢时可将其作为一种评估手段。

Objective To investigate the value of color Doppler ultrasound in evaluating thyroid blood flow grading in hyperthyroidism patients and its correlation with thyroid hormone.Methods A total of 110 cases with hyperthyroidism were admitted from January 2021 to April 2022 in Anyang Third People′s Hospital.A retrospective analysis was conducted.Color Doppler ultrasound was used to diagnose the patients before 131I treatment,and the thyroid hormone indexes of the patients were detected.The blood grading of the thyroid was evaluated by cross-sectional color Doppler ultrasonography,which was divided into grade 1 and grade 2.The relationship between blood grading of thyroid imaging examination and various serum thyroid hormone-related indexes was analyzed.Results There were statistically significant differences in free triiodothyronine(FT3),free thyroxine(FT4),and thyrotropin receptor antibody(TRAb)indexes among patients with grade 1 and grade 2 thyroid blood flow classification(all P<0.05).Moreover,the above indexes were positively correlated with the thyroid blood flow grade(r=0.42,0.41,0.24,P<0.05).The higher the thyroid blood flow grade,the higher the FT3,FT4,and TRAb indexes(P<0.05).Conclusion FT3,FT4,and TRAb indexes in patients with thyroid blood flow grade 1 and grade 2 were statistically significant,and the above indexes were positively correlated with thyroid blood flow grade,there is a correlation between color Doppler ultrasound blood flow grade and thyroid hormone,which can be used as an evaluation method in clinical diagnosis and treatment of hyperthyroidism.
