超声弹性成像联合常规超声诊断甲状腺占位病变的应用价值

摘要 目的 研究超声弹性成像(UE)联合常规超声诊断甲状腺占位病变的应用价值。方法 选取2021年2月至2023年2月我院收治的甲状腺占位病变患者96例,均行UE诊断和常规超声诊断。以术后病理检查结果为“金标准”,统计UE、常规超声单独与联合诊断甲状腺占位病变的诊断结果和诊断效能。结果 本组96例甲状腺占位病变患者经术后病理检查结果确诊良性65例,恶性31例;采用UE诊断出良性52例,恶性25例;采用常规超声诊断出良性44例,恶性20例;采用UE联合常规超声诊断出良性61例,恶性29例;UE联合常规超声诊断灵敏度94%(61/65)、特异度94%(29/31)、准确度94%(90/96)较UE诊断80%(52/65)、81%(25/31)、80%(77/96)和常规超声诊断68%(44/65)、65%(20/31)、67%(64/96)高(P<0.05)。结论 UE联合常规超声诊断应用于甲状腺占位病变中,可提高诊断灵敏度、特异度、准确度,能为临床疾病诊断鉴别提供科学依据。 Objective To investigate the application value of ultrasonic elastography(UE)combined with conventional ultrasound in the diagnosis of thyroid space-occupying lesions.Methods A total of 96 patients with thyroid space occupying lesions admitted to our hospital from February 2021 to February 2023 were diagnosed by UE and routine ultrasound.Using the results of postoperative pathological examination as the"gold standard",the diagnostic results and diagnostic efficacy of UE,conventional ultrasound alone and combined in the diagnosis of thyroid space-occupying lesions were analyzed.Results Of 96 patients with thyroid space occupying lesions,65 cases were benign and 31 cases were malignant.Fifty-two benign cases and 25 malignant cases were diagnosed by UE.Forty-four benign cases and 20 malignant cases were diagnosed by conventional ultrasound.Sixty-one benign cas es and 29 malignant cases were diagnosed by UE combined with conventional ultrasound. The sensitivity, specificity and accuracy of UE combined with conventional ultrasonography were 94% (61/65), 94% (29/31) and 94% (90/96), which were higher than that of UE [80% (52/65), 81% (25/31), 80% (77/96) and 68% (61/65)] and conventional ultrasound [68%(44/65), 65% (20/31), 67% (64/96)] (P<0.05). Conclusion UE combined with conventional ultrasound can improve the sensitivity, specificity and accuracy of the diagnosis of thyroid space occupying lesions, and provide scientific basis for the diagnosis and differentiation of clinical diseases.
