内镜下普通钛夹吻合夹对食管癌术后吻合口瘘的疗效影响评估

Endoscopic Evaluation of the Efficacy of Conventional Titanium Clips for Anastomotic Leakage Following Esophageal Cancer Surgery

目的:探讨内镜下普通钛夹吻合夹对食管癌术后吻合口瘘的疗效及对患者血清指标及预后的影响.方法:回顾性分析我院2018年5月至2022年5月期间实施食管癌手术后发生吻合口瘘的60例患者的临床资料.根据患者治疗手段差异将患者分为对照组(传统的保守治疗,27例)和研究组(内镜下普通钛夹吻合夹治疗,33例).分析对比两组的围术期相关指标、相关检验指标(白细胞计数、C反应蛋白、白蛋白等)、术后1 m内并发症以及生活质量.结果:研究组治疗后住院时间、瘘口愈合时间及引流量均显著少于对照组(P<0.05).研究组治疗后相关检验指标均较对照组改善明显(P<0.05);治疗后研究组的并发症发生率较对照组明显下降(P<0.05);治疗半年后研究组的生活质量显著较对照组高(P<0.05).结论:内镜下普通钛夹夹闭治疗食管癌术后吻合口瘘,能减轻患者的炎性反应程度,改善营养状态,减少并发症,改善患者的日常生活.

Objective:To investigate the efficacy of endoscopic application of common titanium clips for anastomotic leakage following esophageal cancer surgery,and to assess its impact on serum indicators and prognosis.Methods:A retrospective analysis was conducted on clinical data from 60 patients who experienced anastomotic leakage after esophageal cancer surgery at our hospital between May 2018 and May 2022.Based on differences in treatment approaches,patients were categorized into a control group(traditional conservative treatment,27 cases)and a study group(endoscopic application of common titanium clips,33 cases).Perioperative indicators,relevant laboratory tests(Leukocyte count,C-reactive protein,albumin,etc.),complications within one month after surgery,and quality of life were compared between the two groups.Results:In the study group,the length of hospital stay,time to anastomotic healing,and drainage volume were significantly shorter than in the control group(P<0.05).Relevant laboratory indicators in the study group improved significantly compared to the control group(P<0.05).The incidence of complications after treatment was significantly lower in the study group than in the control group(P<0.05).Six months after treatment,the study group exhibited significantly higher quality of life compared to the control group(P<0.05).Conclusion:Endoscopic application of common titanium clips for closure is effective in treating anastomotic leakage following esophageal cancer surgery.It reduces inflammatory responses,improves nutritional status,decreases complications,and enhances patients'daily lives.
