中药热奄包辅助治疗血脉瘀阻型糖尿病足的效果观察

Observation on the Effect of Traditional Chinese Medicine Hot Package in the Adjuvant Treatment of Blood Stasis Type Diabetic Foot

目的:探讨中药热奄包在血脉瘀阻型糖尿病足中的临床疗效。方法:选取2022年4月—2023年3月在河南中医药大学第一附属医院周围血管科进行治疗的血脉瘀阻型糖尿病足患者80例,根据随机数字表法分为对照组40例和实验组40例。对照组予以临床常规治疗,实验组在其基础上辅以中药热奄包干预,比较两组临床疗效、炎症因子水平、疼痛评分、间歇性跛行距离、足背动脉内径变化、血流变化和踝肱指数。结果:实验组总有效高于对照组(P<0.05);实验组高敏C反应蛋白、白细胞介素-6、白细胞介素-12和肿瘤坏死因子-α均低于对照组(P<0.05);实验组疼痛评分和间歇性跛行距离、左右侧足背动脉内径、左右侧足背动脉血流速度和踝肱指数优于对照组(P<0.05)。结论:在治疗血脉瘀阻型糖尿病足中辅以中药热奄包干预的效果良好,可改善患者临床症状,延缓病情发展,为创面愈合创造条件。

Objective: To explore the clinical effect of traditional Chinese medicine hot package in diabetic foot of blood stasis type. Methods: From April 2022 to March 2023, 80 patients with diabetic foot of blood stasis type who were treated in the peripheral vascular department of the First Affiliated Hospital of Henan University of Traditional Chinese Medicine were selected and divided into control group(40 patients) and experimental group(40 patients) according to the random number table method. The control group was given routine clinical treatment, and the experimental group was supplemented with traditional Chinese medicine hot pack intervention on the basis of the control group. The clinical efficacy, inflammatory factor levels, pain scores, intermittent claudication distance, dorsalis pedis artery diameter changes, blood flow changes and ankle brachial index were compared between the two groups. Results: The total effective rate of the experimental group was higher than that of the control group(P<0.05). The high-sensitivity C-reactive protein, interleukin-6, interleukin-12 and tumor necrosis factor-α in the experimental group were lower than those in the control group(P<0.05). The pain score, intermittent claudication distance, left and right dorsalis pedis artery diameter, left and right dorsalis pedis artery blood flow velocity and ankle brachial index in the experimental group were better than those in the control group(P<0.05). Conclusion: In the treatment of blood stasis type diabetic foot, the effect of traditional Chinese medicine hot package intervention is good, which can improve the clinical symptoms of patients, delay the development of the disease, and create conditions for wound healing.
