体质量指数对结肠镜检查的影响

The influence of body mass index on colonoscopy examination

目的观察体质量指数(BMI)对于结肠镜检查的影响。方法选取2021年12月~2022年12月于解放军联勤保障部队第九六二医院进行结肠镜检查的患者299例,根据BMI进行分为低体重组(BMI<18.5 kg/m^(2),n=30)、正常体重组(18.5 kg/m^(2)≤BMI<24 kg/m^(2),n=188)、超重组(24 kg/m^(2)≤BMI<28 kg/m^(2),n=45)和肥胖组(BMI≥28 kg/m^(2),n=36)。分析统计4组患者盲肠插管率、进镜时间、肠道清洁度(BBPS评分)及进镜难度术者评分等相关指标差异。结果低体重组盲肠插管率为93.33%,肠道准备情况良好,进镜时间较长,术者评分较高,操作难度较大;正常体重组盲肠插管率为100%,肠道准备情况均为满意,术者评分较低,操作较为简单,进镜时间最短;超重组盲肠插管率为100%,进镜时间较长,术者评分较低,操作难度较小,肠道准备情况欠佳;肥胖组盲肠插管率为97.22%,肠道准备不佳,进镜时间较长,术者评分最高,操作最为困难。结论正常体重或超重的患者,结肠镜检查的难度较小,肠道清洁度良好,低体重或肥胖的患者,结肠镜检查难度较大,肠道清洁度不佳,因此在结肠镜检查前,应根据患者BMI情况给予更为合理的肠道清洁方案,提高结肠镜检查的安全性与质量。

Objective To observe the impact of body mass index(BMI)on colonoscopy.Methods 299 patients who underwent colonoscopy in our hospital from December 2021 to June 2022 were selected.Based on their BMI,these patients were categorized into underweight group(BMI<18.5 kg/m^(2),n=30),normal group(18.5 kg/m^(2)≤BMI<24 kg/m^(2),n=188),overweight group(24 kg/m^(2)≤BMI<28 kg/m^(2),n=45)and obesity group(BMI≥28 kg/m^(2),n=36).Differences in relevant indicators such as cecal intubation rate,endoscopy time,intestinal cleanliness(BBPS score)and access difficulty(operator score)of patients between groups were analyzed.Results In the Underweight group,the cecal intubation rate is 93.33%,with good intestinal preparation,longer endoscopy time,higher operator score,and increased procedural difficulty.In the normal group,the cecal intubation rate is 100%,In the Overweight group,the cecal intubation rate is 100%,with longer endoscopy time,lower operator score,reduced procedural difficulty,and suboptimal intestinal preparation.In the obesity group,the cecal intubation rate was 97.22%,with poor intestinal preparation,longer endoscopy time,the highest operator score,and the most difficult operation.Conclusion Patients with normal weight or overweight have less difficulty in colonoscopy and good intestinal cleanliness.Patients with low weight or obesity have greater difficulty in colonoscopy and poor intestinal cleanliness.Therefore,it is advisable to develop a more tailored intestinal cleansing plan based on the patient′s BMI prior to colonoscopy in order to enhance the safety and quality of the procedure.
