Clinical management pathways for elderly patients with multimorbidity

共病影响老年人的健康,并使医疗决策更复杂。共病老年患者的管理需要从老年人整体出发,根据患者健康状况确认适合的干预目标;然后依据老年综合评估的结果,通过共同决策,制定适宜的干预方案。该文参照上述策略原则,结合专家共识,制定了共病老年患者管理的临床路径,以便更高效地开展共病老年患者的管理。

Multiple chronic conditions(MCCs)affects the health of the elderly and makes medical decisions more complex.The management of MCCs for the elderly requires a holistic approach and confirmation of appropriate intervention goals based on their health status.According to the results of comprehensive geriatric assessment,we can develop appropriate intervention plans through shared decision-making.We have developed different clinical pathways for the management of elderly patients with MCCs based on the above strategic principles,as well as the consensus of the experts,to provide more efficient advices to the elderly patients with MCCs.
