人工阅片和智能诊断在CT检查肋骨骨折患者中的效能对比

Comparison of Efficacy between Manual Reading and Intelligent Diagnosis in CT Examination of Patients with Rib Fracture

摘要 目的:探讨外伤患者进行胸部多层螺旋CT检查,对比不同级别医师人工阅片和智能软件对肋骨骨折的诊断效能。方法:选取2023年1月至2023年6月我院疑似外伤性肋骨骨折患者150例,均执行多层螺旋CT扫描,并进行薄层及三维后处理重建,以临床及手术综合诊断结果作为金标准,分析比较不同级别医师人工阅片和智能软件对肋骨骨折的诊断结果。结果:经临床及手术综合诊断,发生肋骨骨折患者100例(阳性组),未发生肋骨骨折50例(阴性组)。低年资医师在阳性组诊断正确80例,在阴性组诊断正确40例,准确率为80.0%,漏诊率和误诊率均为20.0%;高年资医师在阳性组诊断正确85例,在阴性组诊断正确45例,准确率为86.7%,漏诊率15.0%,误诊率10.0%;智能软件在阳性组诊断正确95例,在阴性组诊断正确50例,准确率为96.7%,漏诊率5.0%,误诊率0.0%。比较高年资、低年资医师及智能软件诊断结果,高低年资医师两组间准确率和误诊率具有统计学差异(P<0.05),而漏诊率无统计学差异(P>0.05);其他组间比较准确率、误诊率及漏诊率均有统计学差异(P<0.05)。结论:在多层螺旋CT扫描诊断肋骨骨折方面,智能软件比高年资及低年资医师准确率高,效能更好,可为临床推广应用。 Objective:To explore the chest multi-slice spiral CT examination of trauma patients,and compare the diagnostic efficacy of different levels of doctors'manual reading and intelligent software in rib fracture.Methods:150 cases of suspected traumatic rib fracture in our hospital from January 2023to June 2023 were selected,all of them were scanned by multi-slice spiral CT,and reconstructed by thin-slice and three-dimensional post-processing.Taking the comprehensive diagnosis results of clinical and surgery as the gold standard,the diagnosis results of rib fracture by manual reading and intelligent software of different levels of doctors were analyzed and compared.Results:According to the comprehensive diagnosis of clinic and operation,there were 100 cases with rib fracture(positive group)and 50 cases without rib fracture(negative group).80 cases were correctly diagnosed by junior doctors in the positive group and 40 cases in the negative group,with an accuracy rate of 80.0%,and the missed diagnosis rate and misdiagnosis rate were both 20.0%.Senior doctors diagnosed 85 cases correctly in the positive group and 45 cases correctly in the negative group,with an accuracy rate of 86.7%,a missed diagnosis rate of 15.0%and a misdiagnosis rate of 10.0%..The intelligent software correctly diagnosed 95 cases in the positive group and 50 cases in the negative group,with an accuracy rate of 96.7%,a missed diagnosis rate of 5.0%and a misdiagnosis rate of 0.0%.Comparing the diagnosis results of senior doctors,junior doctors and intelligent software,there were statistical differences in accuracy and misdiagnosis rate between the two groups(P<0.05),but there was no statistical dfference in missed diagnosis rate(P>0.05).There were statistical differences in comparison accuracy,misdiagnosis rate and missed diagnosis rate among other groups(P<0.05).Conclusion:In the diagnosis of rib fracture by multi-slice spiral CT scanning,the intelligent sotware has higher accuracy and better efficiency than the doctors with high seniority and low seniority,and can be widely used in clinic.
