葛根芩连汤对湿热下注型溃疡性结肠炎患者TLR4/MyD88信号通路的影响

Effects of Gegen Qinlian Decoction on Levels of TLR4/MyD88 Signaling Pathway in Patientwith Ulcerative Colitis of Dampness-Heat Pouring down Type

目的:分析葛根芩连汤对湿热下注型溃疡性结肠炎(UC)患者Toll样受体4(TLR4)/髓样分化因子88(MyD88)信号通路相关蛋白和下游炎性因子水平的影响。方法:选择100例唐山市中医医院2020年5月—2023年5月收治的UC患者为研究对象,随机分为观察组和对照组,每组50例。对照组进行常规灌肠配合美沙拉嗪肠溶片治疗;观察组在对照组治疗基础上加用葛根芩连汤内服。比较两组治疗效果、治疗前后中医证候积分变化、TLR4、MyD88水平、炎症因子水平以及不良反应发生情况。结果:观察组总有效率高于对照组(P<0.05);治疗后两组中医证候积分下降,且观察组低于对照组(P<0.05);治疗后两组TLR4、MyD88水平下降,且观察组TLR4、MyD88水平低于对照组(P<0.05);治疗后两组炎症因子水平均降低,且观察组炎症因子水平低于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:葛根芩连汤治疗湿热下注型UC患者的疗效显著,可明显改善临床相应症状,调节TLR4/MyD88信号通路相关蛋白和下游炎性因子水平,降低炎症反应,且安全性较高,值得临床推广。

Objective:To analyze the effects of Gegen Qinlian decoction(GQD)on Toll-like receptor 4(TLR4)/myeloid differentiation factor 88(MyD88)signaling pathway related proteins and downstream inflammatory factors in patients with dampness-heat pouring down type ulcerative colitis(UC).Methods:A hundred patients with UC admitted to Tangshan Hospital of Traditional Chinese Medicine from May 2020 to May 2023 were selected as the study objects and randomly divided into observation group and control group,with 50 patients in each group.The control group received conventional enema combined with mesalazine enteric-coated tablets and the observation group was treated the same in addition to GQD orally.The therapeutic effect,the change of traditional Chinese medicine(TCM)syndrome score before and after treatment,the levels of TLR4 and MyD88,the levels of inflammatory factors and the occurrence of adverse reactions were compared between the two groups.Results:The total effective rate of observation group was higher than that of control group(P<0.05).After treatment,the TCM syndrome scores of the two groups decreased,and were lower in the observation group than the control group(P<0.05).The levels of TLR4 and MyD88 in the two groups decreased,and were lower in the observation group than in the control group(P<0.05).The levels of inflammatory factors in both groups decreased,and were better in the observation group than in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Gegen Qinlian decoction has significant efficacy in treating patients with dampness-heat pouring down type UC,can significantly improve clinical symptoms,regulate the levels of TLR4/MyD88 signaling pathway related proteins and downstream inflammatory factors,and reduce inflammatory response,and is safe and worthy of clinical promotion.
