近视人群的角膜生物力学特性及其与CCT、SE、CC、年龄和bIOP的相关性

摘要 目的探讨近视人群的角膜生物力学特性及其与角膜中央厚度(CCT)、等效球镜度数(SE)、角膜曲率(CC)、年龄和生物力学矫正眼内压(bIOP)的相关性。方法近视患者321例(585眼)作为研究对象,采用可视化角膜生物力学分析仪(Corvis ST)测量角膜中央厚度(CCT)、bIOP和11项角膜生物力学参数[第1次压平时间(A1T)、第1次压平速度(A1V)、第1次压平长度(A1L)、第2次压平时间(A2T)、第2次压平速度(A2V)、第2次压平长度(A2L)最大凹陷时间(HC-T)、最大凹陷峰距(HC-PD)、最大凹陷形变幅度(HC-DA)、最大凹陷曲率半径(HC-R)和应力-应变参数(SSI)],采用Pentacam眼前节分析仪测量CCT、3 mm区平面中心曲率(K1)、3 mm区陡中心曲率(K2)、角膜平均曲率(Km);对比两种仪器测量CCT值的差异,分析11项Corvis ST角膜生物力学参数与CCT、SE、CC、年龄和bIOP的相关性。结果两种仪器测量的CCT值存在差异(t=-12.303,P<0.001);除HC-T外,CCT与各生物力学参数均相关(P<0.05);SE与A2T、A2V、SSI呈正相关(P<0.05),与A1T、A1L、HC-PD、HC-DA呈负相关(P<0.05);K1、K2、Km分别与A1V、A2T、HC-T呈正相关(均为P<0.05),与HC-PD、HC-R呈负相关(P<0.001);K2还与HC-DA呈正相关(r=0.081,P=0.050),与SSI呈负相关(r=-0.136、P=0.001);年龄与A1V、A2T、HC-T、HC-DA、SSI呈正相关(P<0.05),与A1T呈负相关(r=-0.110、P=0.008);bIOP与A1T、A1L、A2V、HC-R、SSI呈正相关(P<0.05),与A1V、A2T、HC-T、HC-PD、HC-DA呈负相关(P<0.001)。结论近视人群的角膜生物力学特性与CCT、SE、年龄和bIOP密切相关,角膜厚度大,则生物力学稳定;随着SE增高,角膜生物力学稳定性下降。 Objective To investigate the corneal biomechanical characteristics in myopic individuals and examine their associations with central corneal thickness(CCT),spherical equivalent(SE),corneal curvature(CC),age,and biomechanically-corrected intraocular pressure(bIOP).Methods The study included 321 myopic patients(585 eyes),using Corvis ST for measuring CCT,bIOP,and eleven other corneal biomechanical parameters.Additionally,the Pentacam anterior segment analysis system was used to measure CCT and corneal curvature parameters(K1,K2,Km).The study compared CCT measurements from both devices and analyzed the correlation between Corvis ST biomechanical parameters and CCT,SE,CC,age,and bIOP.Results Significant differences were observed in CCT measurements between the two devices(t=-12.303,P<0.001).Correlations between CCT and all biomechanical parameters were noted(P<0.05),except for HC-T.SE showed positive correlations with A2T,A2V,SSI(P<0.05)and negative correlations with A1T,A1L,HC-PD,HC-DA(P<0.05).K1,K2,Km were positively correlated with A1V,A2T,HC-T(all P<0.05),and negatively correlated with HC-PD,HC-R(P<0.001).Age correlated positively with A1V,A2T,HC-T,HC-DA,SSI(P<0.05)and negatively with A1T(r=-0.110,P=0.008).bIOP showed positive correlations with A1T,A1L,A2V,HC-R,SSI(P<0.05),and negative correlations with A1V,A2T,HC-T,HC-PD,HC-DA(P<0.001).Conclusion Corneal biomechanical properties in myopic individuals are significantly associated with CCT,SE,age,and bIOP,indicating that a thicker cornea correlates with better biomechanical stability,and increasing SE is associated with decreased corneal biomechanical stability.
