层级护理管理在ICU连续性肾脏替代治疗患者中的应用效果

Efficacy of Hierarchical Nursing Management ICU Patients on Continuous Renal Replacement Therapy

目的 探究在ICU连续性肾脏替代治疗患者中应用层级护理管理的临床效果。方法 选取2021年1月至2022年6月鄱阳县人民医院层级护理管理实施前收治的ICU连续性肾脏替代治疗患者41例为对照组,2022年7月至2023年12月层级护理管理实施后的41例为试验组。对比两组护理质量评分、不良事件发生率、并发症发生率、内瘘穿刺一次性成功率及感染发生率。结果层级护理管理后,试验组各项护理质量评分均高于对照组(P <0.05)试验组不良事件发生率低于对照组(4.88%vs. 19.51%,P=0.043);试验组并发症发生率低于对照组(7.32%vs. 24.39%,P=0.034);试验组内瘘穿刺一次性成功率为95.12%(39/41),高于对照组的78.05%(32/41)(P=0.023);感染发生率为2.44%(1/41),低于对照组的14.63%(6/41)(P=0.048)。结论 在ICU连续性肾脏替代治疗患者中应用层级护理管理能明显提高整体护理质量,降低不良事件和并发症发生情况,临床应用成效显著。

Objective To explore the clinical effect of applying hierarchical nursing management in ICU continuous renal replacement therapy patients.Methods A total of 41 cases of ICU continuous renal replacement therapy patients admitted to Poyang County People's Hospital before the implementation of hierarchical nursing management from January 2021 to June 2022 were selected as the control group,and 41 cases after the implementation of hierarchical nursing management from July 2022 to December 2023 were selected as the experimental group.The quality of care scores,adverse event rates,complication rates,one-time success rates of endovascular fistula puncture and infection rates were compared between the two groups.Results After the implementation of hierarchical nursing management,the quality of care scores of the experimental group were higher than those of the control group(P<0.05);the incidence of adverse events in the experimental group was lower than that in the control group(4.88%vs.19.51%,P=0.043);the incidence of complications in the experimental group was lower than that in the control group(7.32%vs.24.39%,P=0.034);and the one-time success rate of endovascular fistula puncture in the experimental group was 95.12%(39/41),higher than 78.05%(32/41)in the control group(P=0.023);the incidence of infection was 2.44%(1/41),lower than 14.63%(6/41)in the control group(P=0.048).Conclusions The application of cascade nursing management in ICU continuous renal replacement therapy patients can significantly improve the overall quality of care,reduce the occurrence of adverse events and complications,and the clinical application is effective.
