头皮脑电图波型在SAH患者发生脑血管痉挛中的应用价值

摘要 目的探讨头皮脑电图波型在蛛网膜下腔出血(SAH)患者发生脑血管痉挛中的应用价值。方法选取2019年1月至2020年1月于甘肃省白银市第一人民医院就诊的86例SAH患者作为研究对象,根据是否发生脑血管痉挛分为发生组(n=25)与未发生组(n=61)。比较两组临床资料,采用多因素Logistic回归分析SAH患者发生脑血管痉挛的影响因素。结果86例SAH患者中,发生脑血管痉挛25例,发生率为29.07%。发生组脑电图波型重度异常占比、乙酰肝素酶水平均高于未发生组,差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,头皮脑电图波型重度异常、乙酰肝素酶水平较高是SAH患者发生脑血管痉挛的独立危险因素(P<0.05)。结论头皮脑电图波型与SAH患者发生脑血管痉挛的发生有关,在发生脑血管痉挛中的应用价值较高。 Objective To explore the application value of scalp electroencephalogram(EEG)wave pattern in cerebral vasospasm in patients with subarachnoid hemorrhage(SAH).Methods 86 patients with SAH admitted to The First People's Hospital of Baiyin from January 2019 to January 2020 were selected as the research subjects and they were divided into the occurrence group(n=25)and the non-occurrence group(n=61)according to the occurrence of cerebral vasospasm.The clinical data between the two groups were compared,and the factors affecting cerebral vasospasm in pa-tients with SAH were analyzed by multivariate Logistic regression.Results Among 86 patients with SAH,25 cases(29.07%)had cerebral vasospasm.The proportion of severe abnormal EEG wave pattern and the level of heparinase in the occurrence group were higher than those in the non-occurrence group,and the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that severe abnormal scalp EEG wave pattern and high level of heparinase were independent risk factors for cerebral vasospasm in SAH patients(P<0.05).Conclusion The scalp elec-troencephalogram wave pattern is related to the occurrence of cerebral vasospasm in SAH patients,and has high application value in the occurrence of cerebral vasospasm.
