血清胃功能四项、幽门螺杆菌与早期胃癌的关系

Relationship between four items of serum gastric function,helicobacter pylori and early gastric cancer

目的探讨血清胃功能四项[胃蛋白酶原(PG)Ⅰ、PGⅡ、PGⅠ/PGⅡ(PGR)、胃泌素-17(G-17)]、幽门螺杆菌与早期胃癌的关系。方法将82例行胃癌筛查人群根据胃镜及病理学检查结果分为早癌组(n=20)和良性组(n=62)。比较两组患者的临床特征。早期胃癌发生的影响因素采用Logistic回归分析。PGⅡ、G-17、PGⅠ、PGR单独及联合检测对早期胃癌的诊断价值采用受试者工作特征(ROC)曲线及曲线下面积(AUC)进行分析。结果两组患者的年龄、幽门螺杆菌抗体、胃癌家族史、吸烟史、PGⅡ、G-17、PGⅠ、PGR比较,差异均有统计学意义(P﹤0.05)。Logistic回归分析结果显示,年龄、幽门螺杆菌抗体、胃癌家族史、吸烟史、PGⅠ、PGⅡ、PGR、G-17是早期胃癌发生的影响因素(P﹤0.05)。ROC曲线显示,PGⅠ、PGⅡ、PGR、G-17联合检测诊断早期胃癌的AUC高于各项指标单独检测。结论年龄、幽门螺杆菌抗体、胃癌家族史、吸烟史、PGⅠ、PGⅡ、PGR、G-17均是早期胃癌发生的影响因素;PGⅠ、PGⅡ、PGR、G-17联合检测对早期胃癌的诊断价值较高。

Objective To explore the relationship between four items of serum gastric function[pepsinogen(PG)I,PGII,PGI/PGII(PGR),gastrin-17(G-17)],helicobacter pylori and early gastric cancer.Method A total of 82 people screened for gastric cancer were divided into early cancer group(n=20)and benign group(n=62)according to the results of gastroscopy and pathological examination.The clinical characteristics of the two groups were compared.The influencing factors of occurrence of early gastric cancer were analyzed by Logistic regression analysis.The diagnostic value of PGII,G-17,PGI,PGR single and combined detection for early gastric cancer was analyzed by receiver operating characteristic(ROC)curve and area under the curve(AUC).Result The age,helicobacter pylori antibody,family history of gastric cancer,smoking history,PGI,PGII,PGR,and G-17 of the two groups were compared,and the differences were statistically significant(P<0.05).Logistic regression analysis showed that age,helicobacter pylori antibody,family history of gastric cancer,smoking history,PGI,PGII,PGR and G-17 were influencing factors of the occurrence of early gastric cancer(P<0.05).ROC curve showed that the AUC of PGI,PGII,PGR,and G-17 combined detection in diagnosis of early gastric cancer were higher than those of each indicators single detection.Conclusion Age,helicobacter pylori antibody,family history of gastric cancer,smoking history,PGI,PGII,PGR,G-17 were the influencing factors of the occurrence of early gastric cancer.The combined detection of PGI,PGII,PGR and G-17 has high diagnostic value for early gastric cancer.
