尼可刹米、纳洛酮联合氨茶碱对急性呼吸衰竭患者血气指标、呼吸功能的影响

Effects of Nikethamide,Naloxone Combined with Aminophylline on Blood Gas Indexes and Respiratory Function in Patients with Acute Respiratory Failure

目的:探讨尼可刹米、纳洛酮联合氨茶碱对急性呼吸衰竭(ARF)患者血气指标、呼吸功能的影响.方法:选取2022年1月—2023年3月单县中心医院收治的ARF患者186例为研究对象,以抽签法随机分为对照组(n=93,给予尼可刹米治疗)与观察组(n=93,在对照组基础上给予纳洛酮联合氨茶碱治疗).比较两组血气指标、呼吸功能指标、症状改善时间和住院时间、药物不良反应.结果:治疗后,两组动脉血氧分压(PaO_(2))水平高于治疗前,动脉血二氧化碳分压(PaCO_(2))水平、pH值低于治疗前,观察组PaO_(2)水平高于对照组,PaCO_(2)、pH值均低于对照组,差异有统计学意义(P<0.05);治疗后,两组第1秒用力呼气容积、用力肺活量水平均高于治疗前,观察组高于对照组,差异有统计学意义(P<0.05);观察组呼吸困难、循环症状、精神症状改善时间及住院时间均短于对照组,差异有统计学意义(P<0.001);两组药物不良反应发生率比较,差异无统计学意义(P>0.05).结论:尼可刹米、纳洛酮联合氨茶碱可改善ARF患者血气、呼吸功能指标,促进患者恢复,安全性较高.

Objective:To investigate the effects of nikethamide,naloxone combined with aminophylline on blood gas index and respiratory function in patients with acute respiratory failure(ARF).Methods:A total of 186 ARF patients admitted to Shanxian Central Hospital from January 2022 to March 2023 were selected as the study objects,and randomly divided into control group(n=93,given nikethamide treatment)and observation group(n=93,given naloxone combined with aminophylline treatment on the basis of control group)by drawing lot.Blood gas index,respiratory function index,symptom improvement time,hospital stay and adverse drug reaction were compared between the two groups.Results:After treatment,the level of arterial partial pressure of oxygen(PaO_(2))was higher than before treatment,the level of arterial partial pressure of carbon dioxide(PaCO_(2))and pH values was lower than before treatment,the PaO_(2) level of observation group was higher than control group,and the PaCO_(2) and pH values were lower than control group,the difference was statistically significant(P<0.05);after treatment,the levels of forced expiratory volume and forced vital capacity in the first second of both groups were higher than before treatment,and the observation group was higher than the control group,the difference was statistically significant(P<0.05);the improvement time of dyspnea,circulatory symptoms,mental symptoms and hospital stay in the observation group were shorter than those in the control group,the difference was statistically significant(P<0.001);there was no significant difference in the incidence of adverse drug reactions between the two groups(P>0.05).Conclusion:Nikethamide and naloxone combined with aminophylline can improve blood gas and respiratory function indexes of ARF patients,promote patient recovery,and have high safety.
