右正中神经电刺激联合艾灸对神外昏迷患者促醒效果的影响

Effect of right median nerve electrical stimulation combined with moxibustion on promoting awakening in patients with extrapsychiatric coma

目的分析右正中神经电刺激联合艾灸对神外昏迷患者促醒效果的影响。方法选取2020年1月至2022年3月于萍乡市中医院神经外科接受治疗的60例昏迷患者作为研究对象,按照随机数字表法分为对照组与研究组,每组30例。对照组实施常规促醒治疗,研究组给予右正中神经电刺激联合艾灸治疗,比较两组治疗效果、格拉斯哥预后量表(GOS)评分、格拉斯哥昏迷量表(GCS)评分及促醒情况。结果研究组治疗总有效率为96.67%,高于对照组的73.33%,差异有统计学意义(P<0.05);两组GOS、GCS评分组内、组间、交互比较差异有统计学意义(P<0.05)。组内比较:治疗14、30 d,两组GOS、GCS评分均高于治疗前,且治疗30 d高于治疗14 d,差异有统计学意义(P<0.05)。组间比较:治疗前,两组GOS、GCS评分比较差异无统计学意义;治疗14、30 d,研究组GOS、GCS评分均高于对照组,差异有统计学意义(P<0.05)。研究组苏醒率高于对照组,苏醒时间短于对照组,差异有统计学意义(P<0.05)。结论右正中神经电刺激联合艾灸在神外昏迷患者促醒中的应用效果显著,可改善患者昏迷状态,缩短促醒时间,值得临床推广应用。

Objective To analyze the effect of right median nerve electrical stimulation combined with moxibustion on promoting awakening in patients with extrapsychiatric coma.Methods A total of 60 coma patients who received treatment in the Neurosurgery department of Pingxiang Hospital of Traditional Chinese Medicine from January 2020 to March 2022 were selected as the study subjects,and they were divided into the control group and the study group according to random number table method,with 30 cases in each group.The control group was given conventional promoting awakening treatment,and the study group was given right median nerve electrical stimulation combined with promoting awakening.The therapeutic effect,Glasgow outcome scale(GOS)score,Glasgow coma scale(GCS)score and promoting awakening were compared between the two groups.Results The total effective rate in the study group was 96.67%,which was higher than 73.33%in the control group,the difference was statistically significant(P<0.05).There were significant differences in GOS and GCS scores between the two groups of groups,time points and interaction(P<0.05).Comparison within group:after 14 and 30 d of treatment,GOS and GCS scores of the two groups were higher than before treatment,and after 30 d of treatment was higher than after 14 d of treatment,the differences were statistically significant(P<0.05).Comparison between groups:before treatment,there were no significant differences in GOS and GCS scores between the two groups;after 14 and 30 d of treatment,GOS and GCS scores in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).The recovery rate in the study group was higher than that in the control group,and the recovery time was shorter than that in the control group,the difference was statistically significant(P<0.05).Conclusion Right median nerve electrical stimulation combined with moxibustion has significant effect in promoting awakening in patients with extrapsychiatric coma,can improve the coma state of patients and shorten the awakening time of patients,which is worthy of clinical promotion and application.
