经阴道彩色多普勒超声联合阴道镜诊断宫颈癌临床探讨

Clinical exploration of transvaginal color Doppler ultrasound combined with colposcopy in the diagnosis of cervical cancer

目的探讨经阴道彩色多普勒超声联合阴道镜诊断宫颈癌的临床价值。方法选取本院90例疑似宫颈癌患者为研究对象。所有患者入院后均接受经阴道彩色多普勒超声与阴道镜检查,比较单一检查方式与联合检查的诊断价值。结果90例疑似宫颈癌患者,经病理学检查最终确诊81例,其余9例患者均为宫颈炎症。阴道镜的特异度、敏感度和准确度、阳性预测率和阴性预测率与经阴道彩色多普勒超声比较差异无统计学意义(P>0.05)。3种检查方式阳性预测率比较差异无统计学意义(P>0.05)。联合检查的特异度、敏感度和准确度、阳性预测率和阴性预测率分别为97.53%、88.89%、87.00%、98.75%和80.00%,明显高于单独阴道镜或者经阴道彩色多普勒超声检查,差异有统计学意义(P<0.05)。结论相比单独采用阴道镜或者经阴道彩色多普勒超声检查,二者联合检查更利于精准发现并诊断宫颈癌,提高诊断符合率,值得临床推广与应用。

Objective To explore the clinical value of transvaginal color Doppler ultrasound combined with colposcopy in the diagnosis of cervical cancer.Methods 90 suspected cervical cancer patients in our hospital were selected as the research subjects.All patients underwent transvaginal color Doppler ultrasound and colposcopy examination after admission,and the diagnostic value of single examination method and combined examination was compared.Results Among the 9o suspected cervical cancer patients,81 were ultimately diagnosed through pathological examination,and the remaining 9 patients were all diagnosed with cervicitis.There was no statistically significant difference(P>0.05)in the specificity,sensitivity,accuracy,positive and negative predictive rates of colposcopy compared to transvaginal color Doppler ultrasound.There was no statistically significant difference in the positive predictive rate among the three examination methods(P>0.05).The specificity,sensitivity,accuracy,positive predictive rate,and negative predictive rate of the combined examination were 97.53%,88.89%,87.00%,98.75%,and 80.00%,respectively,which were significantly higher than those of colposcopy alone or transvaginal color Doppler ultrasound examination,with statistical significance(P<0.05).Conclusion Compared to using colposcopy or transvaginal color Doppler ultrasound alone,the combined examination of the two is more conducive to accurate detection and diagnosis of cervical cancer,improving diagnostic accuracy,and is worthy of clinical promotion and application.
