复盘能力大赛结果分析与模拟教师培训策略的思考

摘要 目的分析2021年上海市住院医师规范化培训教学能力大赛情境模拟复盘能力决赛结果,结合KOLB学习循环原理,为制定更合理的教师培训策略,缩短教师的学习曲线提供依据。方法12位参赛教师,8位评委参与比赛。复盘评估内容分为3大项12小项,分别为整体评估:包括心理安全、正确评估、条理清晰和住院医师参与;教学方法:包括聚焦实践体验、引导反思、形成改进策略和确认学习成效;教师行为:包括避免武断判断、学习负荷适当、使用肢体语言和控场能力。每小项满分为10分,总分120分。结果在3大项中,教学方法获得的评分最低。其中,引导反思、确认学习成效及控场能力的成绩中位数低于6分;正确评估,避免武断判断,学习负荷适当及使用肢体语言的成绩中位数高于7分;心理安全成绩差异最大。结论教师在对教学相关心理学认知方面及如何通过有效的交流访谈,引导住院医师反思,形成新的策略方面仍然存在改进的空间。遵循Kolb学习循环原理,在培训形式上建议调整为以理论结合实践工作坊的形式,并增加实践反馈环节;内容更加精细化、标准化有利于缩短学习曲线。 Objective By analyzing the facilitators'debriefing ability in Year 2021 Shanghai Standardized Residency Training Scenario Based Simulation Competition,and combining with Kolb learning cycle,to provide the basis for more reasonable trainer training strategy and shortening the learning curve.Methods Twelve facilitators and eight judges participated in the competition.The debriefing ability rated items included 3 parts and 12 details.They were general evaluation,including psychological safety,correct assessment,well organization and rensidents involvement;teaching methods,including focusing on experience,guiding reflection,forming improvement plans and confirming learning results;and teacher behavior,including avoiding arbitrary decision,proper learning load,body language and controlling ability.A score of 10 was assigned to every item and the total score was 120.Results The score of teaching method was the lowest in the three parts.Median scores of guiding reflection,forming improvement plans and controlling ability were less than 6.Median scores of correct assessment,avoiding arbitrary decision,proper learning load and body language were more than 7.Great dispersion was observed in psychological safety.Conclusions There is still improving space in teaching psychology,guiding reflection,and forming improvement plans by effective interview.According to Kolb learning cycle,the form of training trainers could be modified to lecture combined with wordshop.Feedback after practicing should be considered.More refined and standardized training content may help to shorten the debriefing learning curve.
