超声引导下平面内与平面外颈内静脉穿刺置管术的应用效果

Application Effect of Ultrasound-guided In-plane and Out-of-plane Internal Jugular Vein Puncture and Catheterization

目的:比较超声引导下平面内与平面外颈内静脉穿刺置管术的应用效果。方法:选取2020年1月—2022年12月于德州市宁津县人民医院行颈内静脉穿刺置管术的患者152例作为研究对象,依据双盲法分为平面内组与平面外组,各76例。平面内组采取超声引导下平面内颈内静脉穿刺置管术,平面外组采取超声引导下平面外颈内静脉穿刺置管术。比较两组患者的穿刺相关指标、并发症发生率及一次穿刺成功率。结果:平面外组穿刺时间短于平面内组,置管时间长于平面内组,穿刺针数少于平面内组,差异有统计学意义(P<0.05)。两组并发症总发生率比较,差异无统计学意义(P>0.05)。平面外组一次穿刺成功率高于平面内组,差异有统计学意义(P=0.002)。结论:超声引导下平面外颈内静脉穿刺置管术的应用效果较好,可缩短穿刺时间,提高一次性穿刺成功率。

Objective:To compare the application effect of ultrasound-guided in-plane and out-of-plane internal jugular vein puncture and catheterization.Methods:A total of 152 patients who underwent internal jugular vein puncture and catheterization in the Ningjin County People's Hospital of Dezhou City from January 2020 to December 2022 were selected as the study subjects.They were divided into in-plane group and out-of-plane group by double-blind method,with 76 cases in each group.The in-plane group received ultrasound-guided in-plane internal jugular vein puncture and catheterization,and the out-of-plane group received ultrasound-guided out-of-plane internal jugular vein puncture and catheterization.The related indexes,incidence of complications,and number of one-puncture successful cases were compared between the two groups.Results:The time of puncture in the out-of-plane group was shorter than that in the in-plane group,the catheterization time in the out-of-plane group was longer than that in the in-plane group,the number of puncture needles in the out-of-plane group was less than that in the in-plane group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the total incidence of complications between two groups(P>0.05).The number of one-puncture successful cases in the out-of-plane group was higher than that in the in-plane group,and the difference was statistically significant(P=0.002).Conclusion:The application of ultrasound-guided out-of-plane internal jugular vein puncture and catheterization has better effects,and shorten the time of puncture,and increase the one-puncture success rate.
