社区2型糖尿病患者胰岛素治疗满意度与注射依从性影响因素分析

Analysis of Factors Influencing Insulin Treatment Satisfaction and Injection Adherence among Community Patients with Type 2 Diabetes

目的:分析社区2型糖尿病患者胰岛素治疗满意度及注射依从性的影响因素。方法:选取2020年1月—2023年9月于上海市浦东新区沪东社区卫生服务中心及上海市浦东新区唐镇社区卫生服务中心行胰岛素治疗的2型糖尿病患者200例作为研究对象。收集患者基本信息,评估其胰岛素治疗满意度和注射依从性,采用Logistic回归模型分析影响患者胰岛素治疗满意度和依从性的因素。结果:根据胰岛素治疗满意度问卷结果将200例研究对象分为满意组(n=141)与不满意组(n=59)。满意组年龄小于不满意组,糖尿病教育率高于不满意组,胰岛素的认知程度优于不满意组,差异有统计学意义(P<0.05)。根据胰岛素注射依从性调查问卷结果将200例研究对象分为依从性好组(n=135)与依从性差组(n=65)。依从性好组年龄小于依从性差组,文化程度、糖尿病教育率、收入高于依从性差组,胰岛素的认知程度优于依从性差组,差异有统计学意义(P<0.05)。Logistic回归分析显示,年龄是患者胰岛素治疗满意度的独立影响因素(P<0.001),年龄、文化程度、糖尿病教育、胰岛素认知程度、收入状况是患者胰岛素注射依从性的独立影响因素(P<0.05)。结论:年龄是2型糖尿病患者胰岛素治疗满意度的独立影响因素,年龄、教育水平、糖尿病知识知晓度、胰岛素认知水平和收入状况是2型糖尿病患者胰岛素注射依从性的独立影响因素,临床应通过针对性的沟通和教育提高其胰岛素治疗满意度及服药依从性。

Objective:To analyze factors influencing satisfaction with insulin treatment and injection adherence among community patients with type 2 diabetes.Methods:A total of 200 patients with type 2 diabetes who received insulin treatment at the Hudong Community Health Service Center in Pudong New Area of Shanghai and the Tangzhen Community Health Service Center in Pudong New Area of Shanghai from January 2020 to September 2023 were selected as the study subjects.Basic information of pa⁃tients were collected.The patients'satisfaction with insulin treatment and injection adherence were assessed.Logistic regression model was used to analyze the factors influencing patients'satisfaction with insulin treatment and adherence.Results:According to the results of the insulin treatment satisfaction questionnaire,200 subjects were divided into satisfactory group(n=141)and unsat⁃isfactory group(n=59).The age in the satisfactory group was lower than that in the unsatisfactory group,the diabetes education rate in the satisfactory group was higher than that in the unsatisfactory group,and the degree of insulin awareness in the satisfactory group was better than that in the unsatisfactory group,and the difference was statistically significant(P<0.05).According to the re⁃sults of the insulin injection adherence questionnaire,200 subjects were divided into good adherence group(n=135)and poor ad⁃herence group(n=65).The age in the good adherence group was lower than that in the poor adherence group,the education level,diabetes education rate and income in the good adherence group were higher than those in the poor adherence group,and the de⁃gree of insulin awareness in the good adherence group was better than that in the poor adherence group,and the difference was sta⁃tistically significant(P<0.05).Logistic regression analysis showed that,age was the independent influencing factor of patients'sat⁃isfaction with insulin treatment(P<0.001),and age,education level,diabetes education,insulin awareness degree and income sta⁃tus were the independent influencing factors of patients'adherence with insulin injection(P<0.05).Conclusion:Age is the indepen⁃dent influencing factor of satisfaction with insulin treatment in patients with type 2 diabetes;while age,educational level,diabetes knowledge,insulin awareness,and income status are the independent influencing factors of adherence with insulin injection in pa⁃tients with type 2 diabetes.In clinical practice,targeted communication and education are required to improve the satisfaction with insulin treatment and medication adherence.
