社区家庭医生签约服务在老年原发性高血压患者中的应用效果

Application Effect of Community Family Doctor Contract Service in Elderly Patients with Essential Hypertension

目的:探讨社区家庭医生签约服务在老年原发性高血压患者中的应用效果。方法:选取2022年1—6月北京市通州区次渠卫生院收治的120例老年原发性高血压患者作为研究对象,采用随机数字表法分为研究组与对照组,各60例。对照组采取常规社区管理方案,研究组在对照组基础上采取社区家庭医生签约服务方案。比较两组血压、血脂及生活质量。结果:治疗后,两组收缩压、舒张压低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05)。研究组血压达标率高于对照组,差异有统计学意义(P<0.001)。治疗后,两组甘油三酯、总胆固醇、低密度脂蛋白胆固醇水平低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05)。研究组血脂达标率高于对照组,差异有统计学意义(P=0.001)。治疗后,两组生理、心理、社会关系、认知功能评分高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05)。结论:社区家庭医生签约服务对老年原发性高血压患者的管理效果良好,可改善患者血压、血脂控制情况及生活质量。

Objective:To explore the application effect of community family doctor contract service in elderly patients with essential hypertension.Methods:A total of 120 elderly patients with essential hypertension admitted to Ciqu Health Center in Tongzhou District of Beijing from January 2022 to June 2022 were selected as the study subjects.They were divided into control group and study group according to random number table method,with 60 cases in each group.The control group adopted conventional community management scheme,and the study group adopted community family doctor contract service scheme on the basis of the control group.The blood pressure,blood lipid and quality of life were compared between two groups.Results:After treatment,the systolic blood pressure and diastolic blood pressure in the two groups were lower than those before treatment,these pressures in the study group were lower than those in the control group,and the difference was statistically significant(P<0.05).The blood pressure qualified rate in the study group was higher than that in the control group,and the difference was statistically significant(P<0.001).After treatment,the levels of triglycerides,total cholesterol and low density lipoprotein cholesterol in the two groups were lower than those before treatment,the levels in the study group were lower than those in the control group,and the difference was statistically significant(P<0.05).The blood lipid qualified rate in the study group was higher than that in the control group,and the difference was statistically significant(P=0.001).After treatment,the scores of physiological,psychological,social relationship and cognitive function in the two groups were higher than those before treatment,and the scores in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:Community family doctor contract service has good management effect in elderly patients with essential hypertension,and can improve blood pressure and blood lipid control as well as quality of life of patients.
