高血压脑出血患者影像学特征与认知功能障碍的相关性研究 被引量:1

Study on correlation between imaging features and cognitive dysfunction in patients with hypertensive cerebral hemorrhage

摘要 目的研究高血压脑出血患者影像学特征与认知功能障碍的相关性。方法选取2020年10月—2021年12月平煤神马医疗集团总医院收治的100例高血压脑出血并发认知功能障碍患者(设为观察组)和100例高血压脑出血无认知障碍患者(设为对照组)作为此次研究对象,收集两组患者一般临床资料、神经影像学资料,并采用蒙特利尔认知评估表(MoCA)和简易精神状态评价量表(MMSE)评估患者的认知功能,痴呆评分量表(CDR)评估患者的痴呆病情严重程度。采用Person相关性分析上述因素与高血压脑出血认知功能障碍的相关性。结果观察组的脑白质病变发生率、脑内微出血数目多于对照组(P<0.05);观察组的MoCA、MMSE评分均低于对照组,其CDR分级严重于对照组(P<0.05);经Person相关性分析显示,高血压脑出血患者的脑白质病变、脑内微出血数目与MoCA、MMSE评分、CDR分级呈正相关(P<0.05)。结论高血压脑出血患者的认知功能障碍发生与脑白质病变、脑内微出血数目密切相关。 Objective To study the correlation between imaging features and cognitive dysfunction in patients with hyper-tensive cerebral hemorrhage.Methods Totally 100 patients with hypertensive cerebral hemorrhage complicated with cognitive impairment(as the observation group)and 100 patients without cognitive impairment of hypertensive cerebral hemorrhage(as the control group)admitted to our hospital from October 2020 to December 2021 were selected as the objects of this study.General clinical data and neuroimaging data of the two groups were collected.The Montreal Cogni-tive Assessment Scale(MoCA)and The Simplified Mental State Assessment Scale(MMSE)were used to assess the cognitive function of the patients,and the dementia score Scale(CDR)was used to assess the severity of dementia.Per-son correlation was used to analyze the correlation between the above factors and cognitive dysfunction of hypertensive cerebral hemorrhages.Results The incidence of white matter lesions and the number of intracerebral microbleeds in the observation group were higher than those in the control group(P<0.05).MoCA and MMSE scores in the observation group were lower than those in the control group,and CDR scores were more serious than those in the control group(P<0.05).Person correlation analysis showed that there was a positive correlation between MoCA,MMSE score and CDR grade and the number of white matter lesions and intracerebral microbleeds in hypertensive cerebral hemorrhagepa-tients(P<0.05).Conclusion The incidence of cognitive dysfunction in patients with hypertensive cerebral hemor-rhage is closely related to white matter lesions and the number of intracerebral microbleeds.
