OSAHS患者外周血NLR、PLR、SII水平与γ-氨基丁酸及褪黑素浓度水平的预测价值研究

Study on the Predictive Value of Peripheral Blood Levels of NLR,PLR,and SII with Levels ofγ-Aminobutyric Acid and Melatonin Concentration in OSAHS Patients

目的探讨阻塞性睡眠呼吸暂停低通气综合征(Obstructive Sleep Apnea Hypopnea Syndrome,OSAHS)、外周血中性粒细胞与淋巴细胞比率(Neutrophil to Lymphocyte Ratio,NLR)、血小板与淋巴细胞比率(Platelet-to-Lymphoccyte Ratio,PLR)、系统性免疫性炎症指数(Systemic Immune-InflAmmation Index,SII)水平与γ-氨基丁酸及褪黑素浓度的预测价值。方法选取2020年7月—2022年12月广西壮族自治区江滨医院收治的60例OSAHS患者和60例健康体检志愿者作为研究对象。比较正常组和OSAHS组的外周血NLR、PLR、SII水平与γ-氨基丁酸及褪黑素浓度差异,并分析各项指标对OSAHS的预测价值。结果与正常组相比,OSAHS组NLR、PLR、SII水平显著提高,γ-氨基丁酸和褪黑素水平显著降低,差异有统计学意义(P均<0.05)。联合预测对OSAHS的预测准确性最高,AUC值为0.976(95%CI:0.955~0.997),灵敏度为95.0%,特异性为90.0%。结论联合检测NLR、PLR、SII水平与γ-氨基丁酸和褪黑素水平,可反映OSAHS的系统性炎症和睡眠质量改变。

Objective To investigate the predictive value of peripheral blood neutrophil to lymphocyte ratio(NLR),platelet-to-lymphocyte ratio(PLR),and systemic immune-inflammation index(SII)levels in relation toγ-aminobu⁃tyric acid and melatonin concentrations in obstructive sleep apnea hypoventilation syndrome(OSAHS).Methods Sixty OSAHS patients admitted to Guangxi Zhuang Autonomous Region Jiangbin Hospital from July 2020 to Decem⁃ber 2022 and 60 healthy physical examination volunteers were selected as study subjects.The differences in periph⁃eral blood NLR,PLR,and SII levels withγ-aminobutyric acid and melatonin concentrations were compared between the normal group and the OSAHS group,and the predictive value of each index for OSAHS was analyzed.Results Compared with the normal group,NLR,PLR,and SII levels were significantly higher andγ-aminobutyric acid and melatonin levels were significantly lower in the OSAHS group,and the differences were statistically significant(all P<0.05).And the joint prediction had the highest predictive accuracy for OSAHS,with an AUC value of 0.976(95%CI:0.955-0.997),sensitivity of 95.0%and specificity of 90.0%.Conclusion The joint detection of NLR,PLR,and SII levels withγ-aminobutyric acid and melatonin levels can reflect the systemic inflammation and sleep quality alterations in OSAHS.
