全髋关节置换术采用神经阻滞复合喉罩全麻对老年患者应激反应及镇痛效果的影响

Effect of Nerve Block Combined with Laryngeal Mask General Anesthesia on Stress Response and Analgesia in Elderly Patients undergoing Total Hip Arthroplasty

目的探究全髋关节置换术采用神经阻滞复合喉罩全麻对老年患者的应用效果。方法选取2021年3月—2022年10月济阳区人民医院收治的100例全髋关节置换术老年患者为研究对象,依据随机数表法分为两组,每组50例,对照组进行喉罩全麻,观察组进行超声引导下神经阻滞联合喉罩全麻,比较两组应激反应指标及视觉模拟评分量表(Visual Analogue Scale,VAS)评分。结果手术30 min及术毕,观察组各项应激反应指标均低于对照组,差异有统计学意义(P均<0.05)。观察组术后1、24 h VAS评分为(1.13±0.43)分、(2.03±0.54)分,均低于对照组,差异有统计学意义(t=12.352,13.558,P均<0.05)。结论神经阻滞复合喉罩全麻是骨折疾病麻醉的重要方法,在老年全髋关节置换术麻醉中效果明显,能够快速减轻应激反应,镇痛效果显著。

Objective To explore the effect of nerve block combined with laryngeal mask general anesthesia in elderly patients with total hip arthroplasty.Methods A total of 100 elderly patients with total hip replacement treated in Ji⁃yang District People's Hospital from March 2021 to October 2022 were selected as the study objects,and divided into two groups with 50 patients in each group according to random number table method.The control group received laryn⁃geal mask general anesthesia,and the observation group received ultrasus-guided nerve block combined with laryn⁃geal mask general anesthesia.Stress response indexes and Visual Analogue Scale(VAS)scores were compared be⁃tween the two groups.Results At 30 min and after operation,the stress response indexes in the observation group were lower than those in the control group,and the differences were statistically significant(both P<0.05).The VAS scores of the observation group were(1.13±0.43)points and(2.03±0.54)points at 1 h and 24 h after surgery,both lower than those of the control group,and the differences were statistically significant(t=12.352,13.558,both P<0.05).Conclusion Nerve block combined with laryngeal mask general anesthesia is an important method for the fracture dis⁃eases,which has obvious effect in the anesthesia of total hip arthroplasty in the elderly,relieve inflammation,and have significant analgesic effect.
