卡前列素氨丁三醇联合缩宫素对产后出血患者血流动力学及出血情况的影响

Effect of Carboprost Tromethamine Combined with Oxytocin on Hemody⁃namics and Bleeding in Patients with Postpartum Hemorrhage

目的探讨卡前列素氨丁三醇联合缩宫素对产后出血患者血流动力学及出血情况的影响。方法选取2022年3月—2023年4月浙江省人民医院毕节医院就诊的100例高危妊娠且产后出血产妇为研究对象,按随机数表法将其分为对照组(n=50)和观察组(n=50)。对照组给予缩宫素治疗,观察组给予卡前列素氨丁三醇联合缩宫素治疗,比较两组经相关药物治疗后血流动力学及出血情况的改善。结果两组治疗前血流动力学指标对比,差异无统计学意义(P>0.05);治疗后,观察组心率、舒张压和收缩压改善情况均优于对照组,差异有统计学意义(P均<0.05)。观察组产后1、2、24 h的出血量较对照组均明显减少,差异有统计学意义(P均<0.05)。观察组不良反应总发生率为2.00%,低于对照组的14.00%,差异有统计学意义(χ^(2)=4.891,P<0.05)。结论对高危妊娠且产后出血产妇给予卡前列素氨丁三醇联合缩宫素治疗干预,可以有效改善产妇血流动力学指标及产后出血情况,同时显著降低不良反应发生率。

Objective To investigate the effect of carboprost tromethamine combined with oxytocin on hemodynamics and bleeding in patients with postpartum hemorrhage.Methods A total of 100 cases of high-risk pregnancy with post⁃partum hemorrhage who attended Bijie Hospital of Zhejiang Provincial People's Hospital from March 2022 to April 2023 were selected as the research objects and divided into control group(n=50)and observation group(n=50)by ran⁃dom number table method.The control group was given oxytocin treatment,and the observation group was treated with carboprost tromethamine combined with oxytocin,the hemodynamic and hemorrhagic improvements of the two groups after treatment with the relevant drugs were compared.Results There was no significant difference in hemodynamic indexes between the two groups before treatment(P>0.05).After treatment,the improvement of heart rate,diastolic blood pressure and systolic blood pressure in the observation group were better than that in the control group,and the differences were statistically significant(all P<0.05).The amount of bleeding at 1,2 and 24 h after delivery in the ob⁃servation group was significantly lower than that in the control group,and the differences were statistically significant(all P<0.05).The total incidence of adverse reactions in the observation group was 2.00%,which was lower than 14.00%in the control group,and the difference was statistically significant(χ^(2)=4.891,P<0.05).Conclusion The therapeutic intervention of carboprost tromethamine combined with oxytocin in women with high-risk pregnancy and postpartum hemorrhage can effectively improve the hemodynamic indexes and postpartum hemorrhage of women,and at the same time significantly reduce the incidence of adverse reactions.
