玻璃纤维桩联合全瓷冠修复在前牙大面积缺损中的应用研究

Application of glass fiber post combined with porcelain crown in repairing large area defect of anterior teeth

目的分析玻璃纤维桩联合全瓷冠修复在前牙大面积缺损中的应用。方法选取2020年9月至2023年3月漯河市中医院收治的120例前牙大面积缺损患者为研究对象,按照随机数字表法分为单一组与联合组,每组60例。单一组予以金属桩钴铬烤瓷冠修复,联合组予以玻璃纤维桩联合全瓷冠修复。比较两组临床疗效、咀嚼效率、牙周状况[龈沟出血指数(SBI)、探诊深度(PD)、临床附着丧失(CAL)]水平、龈沟液情况[碱性磷酸酶水平(ALP)、龈沟液基质金属蛋白酶2(MMP2)、细胞间黏附分子-1(ICAM-1)]水平及并发症发生率。结果联合组治疗总有效率为95.00%(57/60),高于单一组的81.67%(49/60),差异有统计学意义(P<0.05)。修复后,联合组咀嚼效率高于单一组,PD、SBI、CAL水平低于单一组,MMP2、ALP、ICAM-1水平低于单一组,差异有统计学意义(P<0.05)。联合组并发症发生率为5.00%(3/60),低于单一组[16.67%(10/60)],差异有统计学意义(P<0.05)。结论对前牙大面积缺损患者采用玻璃纤维桩联合全瓷冠修复,可提高治疗效果,提高咀嚼效率,减轻对牙周组织和龈沟液的影响,降低并发症发生率,安全性良好。

Objective To analyze the application of glass fiber post combined with porcelain crown in the restoration of large area defect of anterior teeth.Methods A total of 120 patients with large anterior tooth defect in Traditional Chinese Medicine Hospital of Luohe from September 2020 to March 2023 were selected and divided into the combined group and the single group according to random number table method,with 60 cases in each group.The single group was repaired with metal post cobalt-chromium porcelain crown,and the combined group was repaired with glass fiber post combined with all-porcelain crown.The clinical efficacy,masticatory efficiency,periodontal status[gingival crevicular bleeding index(SBI),probing depth(PD),clinical attachment loss(CAL)],gingival crevicular fluid[alkaline phosphatase level(ALP),gingival crevicular fluid matrix metalloproteinase 2(MMP2),intercellular adhesion molecular 1(ICAM-1)]and complication rates were compared between the two groups.Results The total effective rate of combined group was 95.00%(57/60),which was higher than that of single group(81.67%,49/60,P<0.05).After repair,the masticatory efficiency of combined group was higher than that of single group(P<0.05).After repair,the levels of PD,SBI and CAL in combined group were lower than those in single group(P<0.05).After repair,the levels of MMP2,ALP and ICAM-1 in combination group were lower than those in single group(P<0.05).The complication rate of the combined group was 5.00%(3/60),which was lower than that of the single group(16.67%,10/60,P<0.05).Conclusions The combination of glass fiber post and porcelain crown can improve the therapeutic effect,improve the masticatory efficiency,reduce the influence on periodontal tissue and gingival crevicular fluid,and reduce the incidence of complications,with good safety.
