癫痫患者定量脑电图功率谱相关参数与认知功能损伤的相关性研究

Correlation between quantitative electroencephalogram power spectrum related parameters and cognitive impairment in patients with epilepsy

目的分析癫痫(EP)患者定量脑电图(QEEG)功率谱相关参数与认知功能损伤的相关性。方法选取公安县人民医院神经内科自2019年8月至2021年8月收治的130例EP患者纳入观察组,另按1∶1比例选取同期在本院接受体检的130例健康志愿者纳入对照组,比较2组受试者的临床记忆量表(CMS)评分。根据记忆商(MQ)分值将EP患者分为认知功能损伤组(MQ<90分)和认知功能正常组(MQ≥90分)。比较3组受试者的α、θ、β及δ频段百分比功率值,分析QEEG各频段百分比功率值与CMS评分的相关性。结果观察组CMS各维度分值均显著低于对照组,差异具有统计学意义(P<0.05)。EP患者中有54例MQ<90分,纳入认知功能损伤组,余76例MQ≥90分,纳入认知功能正常组。认知功能损伤组α频段百分比功率值显著低于认知功能正常组、对照组,θ频段、δ频段百分比功率值显著高于认知功能正常组、对照组,差异具有统计学意义(P<0.05)。Pearson相关性分析显示,QEEGα频段百分比功率值与MQ呈显著正相关(r=0.338,P<0.05),θ频段、δ频段百分比功率值与MQ呈显著负相关(r=-0.695、-0.724,P<0.05)。结论EP患者QEEG功率谱指标与认知功能损伤存在显著相关性,或可为EP患者认知功能干预提供参考意见。

Objective To analyze the correlation between quantitative electroencephalography(QEEG)power spectrum related parameters and cognitive impairment in patients with epilepsy(EP).Methods A total of 130 patients with EP who were admitted to the Neurology Department at Gongan County People's Hospital from August 2019 to August 2021 were selected as the observation group.Meanwhile,130 healthy volunteers who received health check-ups in the same hospital were selected as the control group.The clinical memory scale(CMS)scores were compared between the two groups.EP patients were divided into a cognitive impairment group[memory quotient(MQ)<90 points]and a cognitive normal group(MQ≥90 points)based on their MQ score.The percentage power values inα,θ,βandδfrequency bands of 3 groups were compared.The correlation between the percentage power value of each QEEG frequency band and CMS was analyzed.Results The scores for different dimensions of CMS in the observation group were significantly lower than those in the control group(P<0.05).Among EP patients,54 patients had MQ score under 90 and they were included in the cognitive impairment group,the other 76 patients with MQ score of 90 or higher were included in the normal cognitive function group.The percentage power value ofαfrequency band in the cognitive impairment group was significantly lower than those in the normal cognitive function group and the control group.The percentage power values ofθandδfrequency bands were significantly higher than those in the normal cognitive function group and the control group(P<0.05).Pearson correlation analysis found that the percentage power value of QEEGαfrequency band was positively correlated with MQ score(r=0.338,P<0.05).The percentage power values ofθandδfrequency bands were negatively correlated with MQ score(r=-0.695,-0.724,P<0.05).Conclusion There is a significant correlation between QEEG power spectrum parameters and cognitive impairment in patients with EP.These parameters may provide reference for cognitive function intervention of patients with EP.
