行为转变理论指导下的呼吸康复训练对老年COPD患者应对方式及行为依从性的影响

The Effects of Respiratory Rehabilitation Training Guided by Behavioral Change Theory on Coping Style and Behavioral Compliance in Elderly Patients with COPD

目的探析行为转变理论指导下的呼吸康复训练在老年慢性阻塞性肺疾病(COPD)中的应用效果。方法观察对象为2023年1月—11月福建省老年医院收治的167例老年COPD患者,按照随机数字表法将其分为两组:A组(83例,常规康复护理)、B组(84例,前者基础上实施行为转变理论指导下的呼吸康复训练),对比两组患者肺功能指标、应对方式评分以及行为依从性。结果在干预3个月后,相较于A组,B组患者用力肺活量(FVC)、第一秒用力呼气容积(FEV1)、第1秒用力呼气容积占用力肺活量百分比(FEV1/FVC)水平均更高(均P<0.05);在干预3个月后,相较于A组,B组患者面对评分更高,回避、屈服评分更低(均P<0.05);B组行为依从率为94.05%,优于A组的80.72%(P<0.05)。结论对老年COPD患者采取行为转变理论指导下的呼吸康复训练,可促使患者以积极的状态来应对疾病,提高患者的训练依从性,改善患者肺功能,促进病情的早日康复。

Objective To explore the effect of respiratory rehabilitation training under the guidance of behavior change theory in the elderly with chronic obstructive pulmonary disease(COPD).Methods A total of 167 elderly patients with COPD admitted to Fujian Geriatric Hospital from January 2023 to November 2023 were divided into two groups according to random number table method:Pulmonary function indexes,coping style scores and behavioral compliance were compared between group A(83 cases,routine rehabilitation nursing)and group B(84 cases,respiratory rehabilitation training under the guidance of behavior change theory was implemented on the basis of the former).Results After 3 months of intervention,compared with group A,patients in group B had higher levels of forced vital capacity(FVC),forced expiratory volume in the first second(FEV1),and forced expiratory volume in the first second as a percentage of forced vital capacity(FEV1/FVC)(all P<0.05);after 3 months of intervention,compared with group A,patients in group B had higher face-to-face scores,and lower avoidance and yielding scores(all P<0.05);Group B had a behavioral compliance rate of 94.05%,which was better than the 80.72%in Group A(P<0.05).Conclusions Respiratory rehabilitation training under the guidance of behavior change theory for elderly patients with COPD can promote patients to deal with the disease in a positive state,improve their training compliance,improve their lung function,and promote the early recovery of the disease.
