血脂及肝功能指标在脂肪肝临床诊断中的价值

The Value of Lipid Index and Liver Function Index in the Diagnosis of Fatty Liver

目的探讨血脂指标及肝功能指标检验在脂肪肝临床诊断中的应用价值。方法选取100例我院2021年1月—2023年1月收治的脂肪肝患者作观察组,另选50名健康体检者作对照组,均采血检测总胆固醇(TC)、三酰甘油(TG)、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)水平,比较两组TC、TG、ALT、AST间差异,并依据脂肪肝严重程度分为轻度、中度及重度,比较三组间TC、TG、ALT、AST差异,绘制ROC曲线,分析TC、TG、ALT、AST及联合检测诊断脂肪肝的临床价值。结果观察组血清TC、TG、ALT、AST水平均较对照组更高(均P<0.05)。重度组血清TC、TG、ALT、AST水平均较中度组、轻度组高(均P<0.05)。绘制ROC曲线显示,TC、TG、ALT、AST及联合检测诊断脂肪肝的曲线下面积分别为:0.754、0.787、0.797、0.799、0.954,联合检测价值最高。结论血脂指标及肝功能指标联合检测可提高脂肪肝诊断效能,便于疾病的早确诊、早治疗。

Objective To explore the value of blood lipid and liver function indicators in the diagnosis of fatty liver.Methods One hundred patients with fatty liver admitted to our hospital from January 2021 to January 2023 were selected as the observation group,and 50 healthy physical examiners were selected as the control group,all of whom had their blood collected to test the levels of total cholesterol(TC),triacylglycerol(TG),alanine aminotransferase(ALT),and aspartate aminotransferase(AST).Compare the differences between TC,TG,ALT,AST between the two groups and classify them into mild,moderate and severe according to the severity of fatty liver,compare the differences of TC,TG,ALT,AST between the three groups,draw ROC curves,and analyze the clinical value of diagnosis of fatty liver by TC,TG,ALT,AST and the combined test.Results Serum TC,TG,ALT and AST levels were higher in the observation group than in the control group(all P<0.05).Serum TC,TG,ALT and AST levels were higher in the severe group than in the moderate and mild groups(all P<0.05).Plotting the ROC curve showed that the areas under the curve of TC,TG,ALT,AST and combined tests for diagnosing fatty liver were 0.754,0.787,0.797,0.799,0.954,respectively,and the combined test had the highest value.Conclusions The combined detection of blood lipid indicators and liver function indicators can improve the diagnostic efficacy of fatty liver,facilitating early diagnosis and treatment of the disease.
