升阳益胃汤对功能性消化不良患者近端胃舒张及胃排空功能的影响

Effect of Shengyang Yiwei Decoction on Proximal Gastric Diastole and Gastric Emptyingin Patients with Functional Dyspepsia

目的:观察升阳益胃汤对功能性消化不良患者近端胃舒张以及胃排空功能的影响。方法:选择103例功能性消化不良患者为研究对象,采用随机数字表法分为观察组52例与对照组51例。对照组采用多潘立酮片治疗,观察组在对照组基础上联合升阳益胃汤治疗。均连续治疗2周。比较2组临床疗效及不良反应,比较2组治疗前后中医证候评分、近端胃舒张功能指标、胃排空功能以及血清胃动素(MTL)、胃泌素(GAS)水平。结果:观察组总有效率94.00%,高于对照组76.47%(P<0.05)。治疗后,2组胃脘胀痛或痞闷、疲乏无力、面色萎黄、嗳气呃逆、纳差食少、大便稀溏等中医证候评分均下降(P<0.05),且观察组低于对照组(P<0.05)。治疗后,2组胃底气体评分均降低(P<0.05),且观察组低于对照组(P<0.05)。治疗后,试餐饮用完毕后10 min(T_(2))、30 min(T_(3))、60 min(T_(4))时,2组近端胃容积均减小(P<0.05),且T_(3)、T_(4)时,观察组近端胃容积小于对照组(P<0.05)。治疗后,2组胃排空率均升高(P<0.05),且观察组高于对照组(P<0.05)。治疗后,2组MTL、GAS均升高(P<0.05),且观察组高于对照组(P<0.05)。结论:升阳益胃汤可有效改善功能性消化不良患者近端胃舒张、胃排空功能,缓解临床症状,提高临床疗效。

Objective:To observe the effect of Shengyang Yiwei Decoction on proximal gastric diastole and gastric emptying in patients with functional dyspepsia.Methods:A total of 103 patients with functional dyspepsia were selected as the study objects and divided into the observation group and the control group by random number table method,with 52 cases and 51 cases in each group respectively.The control group Was treated with Domperidone Tablets,and the observation group was additionally treated with Shengyang Yiwei Decoction based on the treatment of the control group.All patients were treated for 2 weeks.The clinical effects and adverse reactions in the two groups were compared,and the traditional Chinese medicine(TCM)syndrome scores,proximal gastric diastolic function indexes,gastric emptying function and serum levels of motilin(MTL)and gastrin(GAS)were compared before and after treatment between the two groups.Results:The total effective rate was 94.00%in the observation group,higher than that of 76.47%in the control group(P<0.05).After treatment,the TCM syndrome scores of stomach cavity distention and pain or impediment and sealed moistening,fatigue and weakness,withered-yellow complexion,belching and hiccups,poor appetite,and thin and unformed stool were all decreased in the two groups(P<0.05),and the above scores in the observation group were lower than those in the control group(P<0.05).After treatment,the gastric fundus gas scores in both groups were decreased(P<0.05),and the gas score in the observation group was lower than that in the control group(P<0.05).After treatment,10 minutes after trial diet(T_(2)),30 minutes after trial diet(T_(3))and 60 minutes after trial diet(T_(4)),the proximal gastric volume of both groups was respectively decreased(P<0.05),and at T_(3) and T_(4),the proximal gastric volume of the observation group was respectively smaller than that of the control group(P<0.05).After treatment,the gastric emptying rates in both groups were increased(P<0.05),and the rate in the observation group was higher than in the control group(P<0.05).After treatment,the levels of MTL and GAS were increased in both groups(P<0.05),and the two levels in the observation group were higher than those in the control group(P<0.05).Conclusion:Shengyang Yiwei Decoction can effectively improve the proximal gastric diastole and gastric emptying function in patients with functional dyspepsia,relieve clinical symptoms and improve clinical effect.
