通淋解毒化热方结合敏感抗生素治疗急性泌尿系感染临床疗效及对炎症因子的影响

Clinical Efficacy of Tonglin Jiedu Huare Formula(通淋解毒化热方)Combined with Sensitive Antibiotics in Treatment of Acute Urinary Tract Infection and Its Influence on Inflammatory Factors

目的观察通淋解毒化热方结合敏感抗生素治疗急性泌尿系感染的临床疗效及对炎症因子影响。方法选取2020年5月—2022年4月医院收治的91例急性泌尿系感染患者,以随机数字表法分为对照组(45例)和观察组(46例),对照组根据药敏试验予敏感抗生素治疗,观察组在对照组的基础上给予通淋解毒化热方治疗,观察比较两组患者细菌清除率、临床疗效、中医症状积分(小便频急不畅、血尿、小腹拘急、腰部酸痛等)变化、平均尿菌转阴时间、尿路刺激征消失时间及体温恢复正常时间、降钙素原(PCT)和C反应蛋白(CRP)及白细胞计数(WBC)水平等炎症因子指标变化、不良反应、治疗3个月与6个月疾病复发次数。结果观察组患者细菌清除率及总有效率均比对照组患者更高,P<0.05;治疗前两组患者中医症状积分,PCT、CRP、WBC等炎症因子指标比较,P>0.05,治疗后两组患者中医症状积分,PCT、CRP、WBC等炎症因子指标均较治疗前降低,P<0.05,且观察组明显低于对照组,P<0.05;观察组平均尿菌转阴时间、尿路刺激征消失时间及体温恢复正常时间均短于对照组,P<0.05;观察组治疗后3个月与治疗后6个月疾病复发次数均少于对照组,P<0.05;两组均未见明显不良反应发生。结论通淋解毒化热方结合敏感抗生素治疗急性泌尿系感染的临床疗效显著,患者细菌清除率高,症状改善,炎症因子水平降低,无不良反应,值得应用。

Objective To observe the clinical efficacy of Tonglin Jiedu Huare Formula(通淋解毒化热方)combined with sensitive antibiotics in the treatment of acute urinary tract infection and its influence on inflammatory factors.Methods Ninety-one patients with acute urinary tract infection admitted to the hospital from May 2020 to April 2022 were selected and divided into control group(45 cases)and observation group(46 cases)by random number table method.The control group was treated with sensitive antibiotics according to drug sensitivity test,and the observation group was treated with Tonglin Jiedu Huare Formula on the basis of the control group.The bacterial clearance rate,the clinical efficacy,traditional Chinese medicine symptom scores(frequent and slow urination,hematuria,abdominal tightness,waist pain,etc.),the average time for urinary bacteria to turn negative,the time for urinary tract irritation to clear,the time for body temperature to return to normal level,procalcitonin(PCT),C-reactive protein(CRP),white blood cell count(WBC)and other inflammatory factors were observed and compared between the two groups.The adverse reactions and the times of disease recurrence 3 and 6 months after treatment were observed.Results The bacterial clearance rate and total effective rate in the observation group were higher than those in the control group(P<0.05).Before treatment,TCM symptom scores,the levels of PCT,CRP,WBC and other inflammatory factors were not significant between the two groups(P>0.05).After treatment,TCM symptom scores,the levels of PCT,CRP,WBC and other inflammatory factors were decreased between the two groups(P<0.05),and those of the observation group were significantly lower than those of the control group(P<0.05).The average time of urine bacteria turning negative,the time of urinary tract stimulation disappearing and the time of body temperature returning to normal level in the observation group were shorter than those in the control group(P<0.05).The recurrence times of the observation group at 3 and 6 months after treatment were lower than those of the control group(P<0.05).No obvious adverse reactions were observed in the two groups.Conclusion Tonglin Jiedu Huare Formula combined with sensitive antibiotics has significant clinical effect in the treatment of acute urinary tract infection,with high bacterial clearance rate,improved symptoms,reduced levels of inflammatory factors and no adverse reactions.It is worthy of application.
