化痰活络饮配合针刺治疗脑卒中抑郁症疗效及对其血清RBP4和SP表达的影响

Effect of Huatan Huoluo Decoction(化痰活络饮)Combined with Acupuncture on Stroke Depression and Its Effect on Expressions of Serum RBP4 and SP

目的探讨化痰活络饮配合针刺治疗脑卒中抑郁症疗效及对其血清RBP4和SP表达的影响。方法选取2017年3月—2019年3月医院收治的98例脑卒中抑郁症患者,随机分为观察组和对照组,各49例。对照组患者给予针刺治疗,观察组患者在对照组的基础上联合化痰活络饮进行治疗,比较两组患者治疗后临床疗效和治疗前后SDS、HAMD评分、血清RBP4及SP水平表达的影响,以及不良反应发生情况。结果治疗后,观察组临床总有效率为93.88%,显著高于对照组的71.43%(P<0.05)。两组患者SDS评分、HSMD评分较治疗前显著降低,且观察组SDS评分(49.55±8.32)分、HSMD评分(15.39±2.48)分较对照组(57.31±8.46)分、(19.47±2.33)分更低(P<0.05)。两组患者RBP4水平较治疗前显著下降,且观察组RBP4(28.67±5.59)g/L及SP(45.61±10.58)pg/mL较对照组(35.38±5.32)g/L、(50.35±10.69)pg/mL下降更显著(P<0.05)。治疗后,观察组不良反应总发生为2.04%与对照组的4.08%相当(P>0.05)。结论化痰活络饮配合针刺治疗脑卒中抑郁症患者具有显著效果,可有效改善患者抑郁状态,降低SDS、HAMD评分,改善神经功能,调降血清RBP4及SP水平,且不良反应较少,安全性较好,指导临床推广使用。

Objective To explore the curative effect of Huatan Huoluo Decoction(化痰活络饮)combined with acupuncture in the treatment of stroke depression and its effect on the expressions of serum retinol binding protein(RBP4)and serum substance P(SP).Methods A total of 98 stroke depression patients admitted to the hospital from March 2017 to March 2019 were selected and randomly divided into observation group and control group,with 49 cases in each group.The patients in the control group were given acupuncture treatment,and those in the observation group were treated with Huatan Huoluo Decoction on the basis of the control group.The clinical efficacy after treatment and the effects of Self Rating Depression Scale(SDS),Hamilton Depression Scale(HAMD)score and the expressions of serum RBP4 and SP before and after treatment were compared between the two groups.And the occurrence of adverse reactions was observed.Results After treatment,the total clinical effective rate of the observation group was 93.88%,which was significantly higher than 71.43% of the control group(P<0.05).The SDS score and HSMD score of the two groups were significantly lower than those before treatment,and the SDS score of the observation group was(49.55±8.32)points and HSMD score was(15.39±2.48)points which were lower than those of the control group[(57.31±8.46)points and(19.47±2.33)points](P<0.05).The RBP4 levels of the two groups were significantly lower than those before treatment,and the levels of RBP4[(28.67±5.59)g/L]and SP[(45.61±10.58)pg/mL]of the observation group were decreased more significantly compared with those of the control group[RBP4(35.38±5.32)g/L and SP(50.35±10.69)pg/mL](P<0.05).After treatment,the total occurrence of adverse reactions in the observation group was 2.04%,which was equivalent to 4.08% in the control group(P>0.05).Conclusion Huatan Huoluo Decoction combined with acupuncture has a significant effect in the treatment of patients with stroke depression.It can effectively improve the depression condition,reduce the scores of SDS and HAMD,improve the neurological function,lower the expressions of serum RBP4 and SP and has fewer adverse reactions and better safety so as to guide clinical promotion and use.
