体外低能量线性冲击波治疗女性压力性尿失禁的效果分析

Effect analysis of linear,low-intensity extracorporeal shock wave on female stress urinary incontinence

目的 探讨体外低能量线性冲击波对女性压力性尿失禁(SUI)的治疗效果。方法 50例女性压力性尿失禁患者,将探头固定于女性大阴唇外侧,接受2次/周、连续3周的低能量线性冲击波治疗, 10 min/次。每个治疗区域使用900~1600个冲击波,每次治疗产生1800~3200个冲击波,低能量线性冲击波的能量密度为0.09 mJ/mm^(2)。比较患者治疗前及治疗后、治疗后2个月国际尿失禁咨询委员会尿失禁问卷表简表(ICI-Q-SF)评分、尿失禁生活质量问卷(I-QOL)评分、临床症状主观分度。结果 患者治疗前、治疗后及治疗后2个月ICI-Q-SF、I-QOL评分比较差异具有统计学意义(P<0.05)。患者治疗后、治疗后2个月ICI-Q-SF评分4.00(1.00, 5.00)、4.00(1.00, 5.00)分低于治疗前的9.00(7.00,11.00)分, I-QOL评分81.82(76.14, 84.09)、81.82(76.14, 85.23)分高于治疗前的75.57(68.18, 81.82)分,差异具有统计学意义(P<0.05);患者治疗后、治疗后2个月ICI-Q-SF评分、I-QOL评分比较差异无统计学意义(P>0.05)。患者治疗前、治疗后及治疗后2个月Ingelman-Sundberg分度比较差异具有统计学意义(P<0.05)。患者治疗前Ingelman-Sundberg分度轻度38例(76.0%)、中度12例(24.0%);治疗后无症状23例(46.0%)、轻度25例(50.0%)、中度2例(4.0%);治疗后2个月无症状23例(46.0%)、轻度26例(52.0%)、中度1例(2.0%),患者治疗后、治疗后2个月Ingelman-Sundberg分度均优于治疗前,差异具有统计学意义(P<0.05);患者治疗后、治疗后2个月Ingelman-Sundberg分度比较差异无统计学意义(P>0.05)。结论 3周体外低能量线性冲击波治疗女性压力性尿失禁能明显缓解尿失禁症状,降低临床主观分度程度,提高患者生活质量。

Objective To explore the effect of linear,low-intensity extracorporeal shock wave on female stress urinary incontinence(SUI).Methods 50 female patients with stress urinary incontinence were selected.The method is fixing the probe outside of greater lips of pudenda,and conducting three-week treatment of linear,low-intensity extracorporeal shock wave twice a week,10 min for every treatment.It will use 900-1600 impulse waves on every treatment area,and 1800-3200 impulse waves for each treatment.The energy density of low-energy linear impulse wave was 0.09 mJ/mm^(2).The International Consultation on Incontinence Questionnaire Short Form(ICI Q-SF)score,Incontinence Quality of Life Questionnaire(I-QOL)score and subjective classification of clinical symptoms were compared before treatment,immediately after treatment and 2 months after treatment.Results There were significant differences in ICI Q-SF score and I-QOL score before treatment,immediately after treatment and 2 months after treatment(P<0.05).Immediately after treatment and 2 months after treatment,ICI Q-SF scores were 4.00(1.00,5.00)and 4.00(1.00,5.00)points,which were lower than 9.00(7.00,11.00)points before treatment;I-QOL scores were 81.82(76.14,84.09)and 81.82(76.14,85.23)points,which were higher than 75.57(68.18,81.82)points before treatment.The difference was statistically significant(P<0.05).The difference in the comparison of ICI-Q-SF score and I-QOL score of patients immediately after treatment and 2 months after treatment was not statistically significant(P>0.05).The difference was statistically significant(P<0.05)when comparing the Ingelman-Sundberg classification of patients before treatment,immediately after treatment and 2 months after treatment.Before treatment,Ingelman-Sundberg classification were mild in 38 cases(76.0%)and moderate in 12 cases(24.0%).After treatment,23 cases(46.0%)were asymptomatic,25 cases(50.0%)were mild,and 2 cases(4.0%)were moderate.2 months after treatment,23 cases(46.0%)were asymptomatic,26 cases(52.0%)were mild,and 1 case(2.0%)was moderate.Ingelman-Sundberg classification immediately after treatment and 2 months after treatment were better than those before treatment,and the difference was statistically significant(P<0.05).There was no significant difference in Ingelman-Sundberg classification immediately after treatment and 2 months after treatment(P>0.05).Conclusion 3-week linear,low-intensity extracorporeal shock wave for female stress urinary incontinence significantly can relieve urinary incontinence symptoms,reduce the subjective classification of clinical symptoms,and improve patients'quality of life.
