盐酸氨溴索联合糖皮质激素对慢性鼻窦炎伴鼻息肉患者术后症状积分及炎症反应的影响

Effect of ambroxol hydrochloride combined with glucocorticoid on postoperative symptom score and inflammatory response in patients with chronic sinusitis and nasal polyps

目的探讨盐酸氨溴索联合糖皮质激素对慢性鼻窦炎伴鼻息肉患者术后症状积分及炎症反应的影响。方法选取2020年9月至2022年8月于我院行鼻内镜手术治疗的60例慢性鼻窦炎伴鼻息肉患者,根据随机数字表法将患者分成两组,每组30例。对照组应用糖皮质激素,观察组在此对照组基础上加以盐酸氨溴索治疗,持续4周。比较组间临床疗效、症状积分、炎症反应及不良反应。结果观察组临床总有效率高于对照组,有统计学差异(P<0.05);治疗前,两组症状积分(鼻塞、流涕、嗅觉障碍)及炎症反应[降钙素原(PCT)、白细胞介素-6(IL-6)]比较,差异无统计学意义(P>0.05);治疗后,观察组鼻塞、流涕、嗅觉障碍评分及PCT、IL-6水平均低于对照组,有统计学差异(P<0.05);观察组不良反应发生比较,差异无统计学意义(P>0.05)。结论在慢性鼻窦炎伴鼻息肉患者中采用盐酸氨溴索结合糖皮质激素治疗效果确切,能降低症状积分,减轻炎症反应,且安全性高。

Objective To investigate the effect of ambroxol hydrochloride combined with glucocorticoid on postoperative symptom score and inflammatory response in patients with chronic sinusitis and nasal polyps.Methods From September 2020 to August 2022,60 patients with chronic sinusitis and nasal polyps who underwent nasal endoscopic surgery in our hospital were selected and divided into two groups according to the random number table method,with 30 cases in each group.The control group was treated with glucocorticoid,and the observation group was treated with ambroxol hydrochloride on the basis of the control group for 4 weeks.The clinical efficacy,symptom score,inflammatory reaction and adverse reactions were compared between groups.Results The total effective rate of the observation group was higher than that of the control group,with statistical difference(P<0.05).Before treatment,there was no significant difference in symptom scores(nasal obstruction,runny nose,olfactory dysfunction)and inflammatory reactions(procalcitonin(PCT),interleukin-6(IL-6))between the two groups(P>0.05).After treatment,the scores of nasal obstruction,runny nose,olfactory disorder,PCT and IL-6 levels in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions in the observation group(P>0.05).Conclusion Ambroxol hydrochloride combined with glucocorticoid is effective in the treatment of chronic sinusitis with nasal polyps,which can reduce the symptom score and inflammation,and has high safety.
