CT检查鉴别诊断鼻腔鼻窦内翻性乳头状瘤与慢性鼻窦炎鼻息肉的临床应用效果

Clinical application of cT in differential diagnosis of inverted papilloma of nasal cavity and paranasal sinuses and chronic sinusitis and nasal polyps

目的 评价CT检查对慢性鼻窦炎鼻息肉、鼻腔鼻窦内翻性乳头状瘤的鉴别诊断价值。方法 选择2021年10月至2022年10月期间在本院就诊的30例慢性鼻窦炎鼻息肉患者、30例鼻腔鼻窦内翻性乳头状瘤患者,分别设为对照组和研究组。全部采取CT检查。对比两组的CT影像表现。结果 研究组的病灶全部(100%)分布在左侧鼻腔,对照组的病灶主要分布在双侧鼻腔(70%),少数分布在左侧(13.33%)或右侧(16.67%)的单侧鼻腔中。对照组中发生下鼻甲吸收变薄者有14例(46.67%),30例(100%)均存在部分筛板吸收不完全问题;研究组中30例(100%)患者均存在下鼻甲吸收破坏,21例(70%)存在中鼻甲吸收破坏,7例(23.33%)存在筛窦骨质吸收破坏。结论 CT检查可以通过了解病灶分布情况和侵蚀破坏情况来鉴别诊断慢性鼻窦炎鼻息肉、鼻腔鼻窦内翻性乳头状瘤。

Objective To evaluate the value of CT in differential diagnosis of chronic sinusitis and nasal polyps and inverted papilloma of nasal cavity and paranasal sinuses.Methods From October 2021 to October 2022,30 patients with chronic sinusitis and nasal polyps and 30 patients with inverted papilloma of nasal cavity and paranasal sinuses were selected as the control group and the study group,respectively.All patients were examined by CT.CT images of the two groups were compared.Results All the lesions in the study group(100%)were distributed in the left nasal cavity,while the lesions in the control group were mainly distributed in the bilateral nasal cavity(70%),and a few were distributed in the unilateral nasal cavity on the left(13.33%)or right(16.67%).In the control group,14 cases(46.67%)had thinner inferior turbinate,and 30 cases(100%)had incomplete ethmoid plate absorption;In the study group,30 patients(100%)had inferior turbinate absorption and destruction,21 patients(70%)had middle turbinate absorption and destruction,and 7 patients(23.33%)had ethmoid sinus bone absorption and destruction.Conclusion CT can differentiate chronic sinusitis and nasal polyps from inverted papilloma of nasal cavity and paranasal sinuses by understanding the distribution of the lesions and the erosion and destruction.
